白蛋白系从健康人血浆中分离提取,经艾滋病抗体检测阴性及60℃10小时加温灭活肝炎病毒制成.已广泛用于临床。文献报道,输注白蛋白后发生变态反应率为0.11%,总反应率为0.47~1.58%,(包括荨麻疹、寒战、发热和低血压等),多数症状轻微,经冶疗可恢复.而导致过敏性休克死亡者较罕见,现将静滴白蛋白致过敏性休克死亡1例报告如下. 病例介绍 患者,男,64岁,干部,因咳嗽,咳痰,气急,胸痛2月,于1992年11月16日入院,诊断:左侧肺炎、左侧陈旧性胸膜炎.患者对键霉毒有过敏史.给予消炎、抗痨、对症、支持等综合治疗一个月后病情基本稳定.12月17日11时予以10%的白蛋白100毫升静滴,每分钟20滴,输前肌注非那根25毫克.10分钟后病人突然出现寒战,呼吸急促,当时考虑是输液反应,立即停止静滴,吸氧、肌注非那根25毫克,安痛定2毫升。40分钟后,查体温39.5℃,脉搏130次/分,呼吸30次/分,血压由16/8kPa下降至11/6kPa,患者神志清、头晕、恶心、心悸、气短、口唇紫绀、脉搏微弱.医护人员立
