目的探讨128层螺旋CT在冠心病患者冠状动脉不同程度狭窄中的临床诊断价值。方法对2009年5月至2011年5月收治的冠心病患者40例采用德国西门子128层螺旋CT进行检查,并与选择性冠状动脉造影检查结果进行对照,评价128层螺旋CT冠状动脉造影的准确性。结果在可评价的265支血管节段中,血管正常182支,狭窄83支。CAG和MSCT对冠脉狭窄程度≥50%的冠脉总节段数经统计学处理,差异无统计学意义(P>0.05)。敏感性为97.59%(81/83),特异性为95.60%(174/182),阴性预测值为98.90%(180/182),阳性预测值为95.18%(79/83),准确性为96.23%(255/265)。结论 128层螺旋CT是一种无创的成像方法,能清晰显示冠状动脉管壁的钙化及软斑块和血栓,并可利用智能血管分析软件测得管腔的狭窄程度,在一定程度上可替代常规的选择性冠状动脉造影。
Objective To investigate the value of 128-slice spiral CT in the diagnosis of coronary stenosis in patients with coronary heart disease. Methods Forty patients with coronary heart disease admitted to our hospital from May 2009 to May 2011 were examined by Siemens 128-slice spiral CT and compared with selective coronary angiography to evaluate the accuracy of 128-slice spiral CT coronary angiography Sex. Results Of the 265 evaluable segments of blood vessels, 182 were normal and 83 were stenotic. CAG and MSCT of coronary stenosis ≥ 50% of the total number of segments of the coronary artery by statistical methods, the difference was not statistically significant (P> 0.05). The sensitivity was 97.59% (81/83), the specificity was 95.60% (174/182), the negative predictive value was 98.90% (180/182), the positive predictive value was 95.18% (79/83) and the accuracy was 96.23 % (255/265). Conclusion 128-slice spiral CT is a noninvasive imaging method, which can clearly show the calcification and soft plaque and thrombus in the coronary artery wall. The degree of stenosis of the lumen can be measured by the intelligent blood vessel analysis software and can be replaced to some extent Conventional selective coronary angiography.
