目的探讨三七总皂甙对梗阻性黄疸心肌损伤中的保护机制。方法将日本大耳白兔48只,雌雄各半分为2组,每组24只。A组:胆总管接扎组,B组:胆总管结扎+三七总皂甙组,建立实验动物模型。所有实验动物均在相同条件下饲养。采用免疫组化法检测心肌组织中bax、bcl-2蛋白的表达情况。结果A组动物在胆道梗阻后,心肌组织可见bcl-2和bax蛋白表达,bax蛋白在梗阻14d达高峰,此后开始下降;而bcl-2蛋白在梗阻21d达高峰;B组动物心肌组织bax蛋白表达较同时相A组弱,而bcl-2蛋白强于A组,B组凋亡指数值较A组降低。结论(1)梗阻性黄疸所致的心肌组织病理变化可能与凋亡程序的启动有关。(2)三七总皂甙对梗阻性黄疸心肌组织损伤有明显的保护作用,其机制可能与上调心肌组织bcl-2蛋白、下调bax蛋白有关。
Objective To investigate the protective mechanism of Panax notoginseng saponins in myocardial injury of obstructive jaundice. Methods 48 Japanese white rabbits, male and female, were divided into 2 groups, 24 in each group. Group A: common bile duct ligation group, group B: common bile duct ligation + panax notoginseng group, establish an experimental animal model. All experimental animals were kept under the same conditions. The expression of bax and bcl-2 protein in myocardial tissue was detected by immunohistochemistry. Results After bile duct obstruction in group A, bcl-2 and bax protein expression was observed in myocardium. Bax protein peaked at 14 days after obstruction and then began to decline. bcl-2 protein peaked at 21 days after obstruction; group B animals had myocardial bax protein. The expression was weaker than that of the same phase A group, but the bcl-2 protein was stronger than that of the A group. The apoptotic index value of the B group was lower than that of the A group. Conclusion (1) Myocardial pathological changes caused by obstructive jaundice may be related to the initiation of apoptotic procedures. (2) Panax notoginseng saponins has obvious protective effect on myocardial tissue damage of obstructive jaundice. Its mechanism may be related to up-regulation of bcl-2 protein and down-regulation of bax protein in myocardial tissue.
