“3+2”助理全科医生在社区临床实践的探索

目的讨论“3+2”助理全科医生在社区临床实践的培养方式、优化教学内容,为进一步提高“3+2”助理全科医生在社区临床实践的培训质量提供参考。方法对北京市2014级“3+2”助理全科医师规范化培训学员和学习全科医学概论课程并参加社区临床实践的60名学员;以及基层实践基地24名带教老师,采用问卷调查。结果①绝大部分学员认为有必要采取桌边教学和理论教学一体化的方式来进行基层实践,认为通过基层实践加深了对课程的感兴趣程度(很感兴趣66.67%,一般感兴趣29.8%),②大多数学员与带教师资进行了很好交流(70.2%),③认为效果也比较明显(82.5%),④对此次实践方式感到满意(总体评价为优的占93%)。⑤同时针对调差问卷分析可以得出,学员对师资的教学能力及实践基地的教学建设提出了更高的要求。结论全科医学的基层实践是学员为巩固所学的全科理论知识而进行的再实践,是培养和提高学员业务能力的重中之重,采取诊桌边教学和理论教学一体化的方式,通过增加学员参与诊疗过程,增加学员与老师、学员与患者之间的互动等手段进行教学和实践,能够进一步提高基层实践质量,最终让学员深入理解全科医学的内涵,真正做到居民的实用型“健康守门人”。 Objective To discuss the training mode of “3 + 2 ” assistant general practitioners in community clinical practice and to optimize the teaching content, so as to provide reference for further improving the training quality of “3 + 2 ” assistant general practitioners in community clinical practice. Methods A total of 60 trainees from Beijing 2014 “3 + 2 ” assistant general practitioners were trained on the standardization of general medical courses and community clinical practice; and 24 teaching staff at the grass-roots practice base were investigated by questionnaire . Results ① Most students considered it necessary to adopt the integrated approach of table-top teaching and theoretical teaching to conduct grass-roots practice. They deepened their interest in the curriculum through grass-roots practice (66.67% are very interested and 29.8% are generally interested) , ② most of the trainees communicated well with teachers and teachers (70.2%), ③ considered the results to be more obvious (82.5%), and (4) were satisfied with the practice (93% overall). ⑤At the same time, according to the analysis of the transfer questionnaire, it can be concluded that the trainees put forward higher requirements for the teaching ability of teachers and the teaching construction of the practice base. Conclusion Practice at the grassroots level of GP is a re-practice of trainees in order to consolidate the general knowledge they have learned. It is the most important task for cultivating and improving their abilities in business operation. Taking the combination of clinic-side teaching and theoretical teaching, Teaching and practice can be further enhanced by increasing the participation of trainees in the process of diagnosis and treatment, increasing the interaction between trainees and teachers, trainees and patients, and ultimately enabling trainees to further understand the meaning of general medicine and truly realize the practicality of residents Type “health gatekeeper ”.
