目的探讨超声Graf法在婴儿发育性髋关节发育不良筛查中的应用价值。方法回顾性分析596例婴幼儿(1 192个髋关节)应用超声Graf法检査的结果及临床资料。结果 596例婴儿1 192个髋关节中,正常髋关节1 154个(96.8%),其中Ⅰa型635个,Ⅰb型404个,Ⅱa型106个,Ⅱb型9个;异常髋关节38个,其中Ⅱc型18个,D型8个,Ⅲa型10个,Ⅳ型2个;正常髋关节α角[(63.7±4.7)°]大于异常髋关节[(45.1±4.7)°],β角[(61.1±5.4)°]小于异常髋关节[(68.4±8.4)°],差异均有统计学意义(P<0.05);106个Ⅱa型髋关节和9个Ⅱb型髋关节,复査3次后均转为Ⅰ型,未出现明显关节活动异常;38个异常髋关节经Pavlik约束带或石膏固定等治疗均恢复正常。结论超声Graf法在婴儿髋关节发育异常筛査中具有重要价值,并可用于临床随访及治疗监测。
Objective To investigate the value of Graf method in the screening of developmentally impaired hip dysplasia in infants. Methods The results and clinical data of 596 infants and young children (1 192 hips) with Graf method were analyzed retrospectively. Results Of the 1 192 hip joints in 596 infants, 1 154 (96.8%) had normal hip joints, of which 635 were type Ⅰa, 404 were type Ⅰb, 106 were type Ⅱa, 9 were type Ⅱb and 38 were dysplasia. Among them, 18 in type Ⅱc, 8 in type D, 10 in type Ⅲa and 2 in type Ⅳ. The α angle of normal hip was (63.7 ± 4.7) ° greater than that of abnormal hip joint (45.1 ± 4.7) ° and β angle [( 61.1 ± 5.4 ° °] was less than that of the abnormal hip joint [(68.4 ± 8.4) °], with significant difference (P <0.05); 106 Ⅱa hip joints and 9 Ⅱb hip joints Were changed to type I, no significant abnormalities in joint activity; 38 abnormal hip joint by Pavlik restraint or plaster fixation and other treatment were returned to normal. Conclusion Ultrasound Graf method has important value in the screening of abnormal hip dysplasia in infants and can be used in clinical follow-up and treatment monitoring.
