本文对a_1-抗胰蛋白酶异质体的检测方法(小扁豆凝集素亲合交叉免疫电泳法)进行改进。改进后的方法操作流程短、成本低、结果清晰、操作简便、不需预测a_1-抗胰蛋白酶总浓度,适合于临床广泛应用。本文用该法对原发性肝癌、转移性肝癌,其它器官癌肿、肝硬化及正常人血清中的a_1-抗胰蛋白酶异质体进行分析,发现小扁豆凝集素结合型a_1-抗胰蛋白酶百分比(LCA-R-AAT%)原发性肝癌组明显高于其它各组,以LCA-R-AAT(%)>25%为阳性标准,则原发性肝癌诊断的敏感性、特异性、准确性分别为80.6%、86.2%和84.3%,与小扁豆凝集素结合型甲胎蛋白联合检测,可使原发性肝癌诊断的敏感性提高至100%(P<0.05),特异性和准确性分别为82.8%和89.9%(P均>0.05),表明LCA-R-AAT(%)的检测对原发性肝癌的诊断及鉴别诊断有一定的价值。
In this paper, the detection method of a_1-antitrypsin heterogeneity (lentil agglutinin affinity cross-immunoelectrophoresis) was improved. The improved method has short operating procedures, low cost, clear results, and easy operation. It does not need to predict the total concentration of a_1-antitrypsin, which is suitable for a wide range of clinical applications. In this study, we analyzed the a_1-antitrypsin heteroplasms in primary liver cancer, metastatic liver cancer, other organ cancers, liver cirrhosis and normal human serum, and found that lentil agglutinin-binding a_1-antitrypsin Percentage (LCA-R-AAT%) in primary hepatocellular carcinoma was significantly higher than that in other groups, with LCA-R-AAT (%) > 25% being the positive criterion, then the sensitivity, specificity, Accuracy was 80.6%, 86.2%, and 84.3%, respectively. Combined with lentin-binding alpha-fetoprotein, the sensitivity of primary liver cancer diagnosis was improved to 100% (P<0.05). Specificity and accuracy The rates were 82.8% and 89.9% respectively (all P>0.05), indicating that the detection of LCA-R-AAT (%) has certain value in the diagnosis and differential diagnosis of primary liver cancer.
