目的探讨下肢血管介入在糖尿病下肢血管病变并发糖尿病足临床治疗中的效果。方法分析2011年1月至2015年3月大同市第五人民医院收治的80例糖尿病下肢血管病变患者,随机分为2组:观察组(40例)和对照组(40例)。观察组实施下肢血管腔内介入治疗,对照组行开放手术治疗(对病变血管实施血管内膜剥脱术或血管移植治疗),比较2组患者的手术时间、术中出血及术后住院时间,并统计治疗过程中发生的血管相关并发症。结果观察组成功治疗39例,成功率为97.5%;对照组成功治疗20例,成功率为50.0%,2组相比差异有统计学意义(χ2=20.920,P<0.05)。观察组手术时间短于对照组(P<0.05),术中出血少于对照组(P<0.05),术后住院时间短于对照组(P<0.05),观察组血管破裂、血栓栓塞及血肿形成等相关并发症总发生率明显低于对照组(P<0.05)。结论针对糖尿病下肢血管病变患者使用下肢血管腔内介入治疗,能有效缩短手术时间、减少术中出血且手术并发症少,有利于患者早期康复出院。
Objective To investigate the effect of lower extremity vascular intervention in the clinical treatment of diabetic lower extremity vascular disease complicated with diabetic foot. Methods 80 patients with diabetic lower extremity vascular lesions admitted to the Fifth People’s Hospital of Datong City from January 2011 to March 2015 were randomly divided into two groups: observation group (40 cases) and control group (40 cases). The observation group received interventional therapy of lower extremities vascular intervention, and the control group underwent open surgery (vascular endarterectomy or vascular grafting on diseased vessels). The operation time, intraoperative bleeding and postoperative hospital stay were compared between the two groups Statistics of vascular complications occurred during treatment. Results In the observation group, 39 cases were successfully treated with a success rate of 97.5%. In the control group, 20 cases were successfully treated with a success rate of 50.0%. There was significant difference between the two groups (χ2 = 20.920, P <0.05). The operation time in the observation group was shorter than that in the control group (P <0.05), the intraoperative bleeding was less than the control group (P <0.05), and the postoperative hospital stay was shorter than that in the control group (P <0.05). Conclusion The use of lower extremity endovascular interventional therapy in patients with diabetic lower extremity vascular lesions can shorten the operation time, reduce intraoperative bleeding and fewer complications, which is conducive to the early rehabilitation of patients.
