目的探讨磁共振序列在胎儿磁共振成像(MRI)中的临床应用价值。方法对2013年6月-2014年6月来院检查的87例中晚期孕妇同时行平衡式快速梯度回波(B-FFE)、单次激发快速自旋回波(SS-TSE)、二维小角度T1WI及扩散加权成像(DWI),根据图像质量、图像伪影和信号特点分析各序列在胎儿MRI中的应用。结果 87例胎儿MRI检查中胼胝体发育不全11例,侧脑室扩张21例,颅后窝池增宽19例,脑室旁囊肿3例,脑实质出血3例,眼球发育异常2例,腹部肿瘤4例,隔离肺1例,心脏畸形1例,蛛网膜囊肿4例,脑穿通畸形合并脑回畸形1例,胼胝体发育不全合并脂肪瘤1例,右肺发育不良1例,脑回畸形2例,正常13例。经比较,B-FFE和SS-TSE两种序列都能清楚显示胎儿各器官的解剖结构和异常病变,但SS-TSE对检查中呼吸伪影较敏感;SS-TSE可用于黑血序列鉴别隔离肺、显示血管与肿块的关系、补充心脏成像;T1WI主要用于出血、脂肪性、肿瘤性病变信号判断;DWI用于鉴别颅内蛛网膜囊肿或表皮样囊肿。结论 B-FFE、SS-TSE、T1WI及DWI序列是诊断胎儿发育异常的基本序列。B-FFE和SS-TSE均能清楚显示胎儿器官的解剖结构和异常病变;SS-TSE作为黑血序列可用于寻找血管与肿瘤的关系;T1WI对于诊断出血、脂肪病变有优势;DWI鉴别缺血缺氧性疾病或蛛网囊肿有很大价值。因此,在胎儿磁共振检查中,4种系列同时应用不易遗漏病灶。
Objective To investigate the clinical value of magnetic resonance (MR) sequences in fetal magnetic resonance imaging (MRI). Methods A total of 87 middle-late-stage pregnant women examined in our hospital from June 2013 to June 2014 underwent simultaneous rapid B-FFE, SS-TSE, T1WI and diffusion-weighted imaging (DWI). The application of each sequence in fetal MRI was analyzed according to image quality, image artifacts and signal characteristics. Results 87 cases of fetal MRI examination of corpus callosum hypoplasia in 11 cases, lateral ventricle expansion in 21 cases, 19 cases of cranial hedge fundus broadening, 3 cases of ventricular cyst, 3 cases of parenchymal hemorrhage, abnormal eyeball development in 2 cases, 4 cases of abdominal tumors , Isolated lung in 1 case, cardiac malformation in 1 case, arachnoid cyst in 4, penetrated deformity with brain deformity in 1 case, corpus callosum hypoplasia complicated with lipoma in 1 case, right lung dysplasia in 1 case, brain deformity in 2 cases, normal 13 cases. However, SS-TSE was more sensitive to respiratory artifacts during the examination. SS-TSE could be used to identify and isolate black blood cells Lung, showing the relationship between blood vessels and tumor, heart imaging; T1WI is mainly used for bleeding, fatty, tumor lesions signal judgment; DWI is used to identify intracranial arachnoid cysts or epidermoid cysts. Conclusion B-FFE, SS-TSE, T1WI and DWI sequences are the basic sequences for diagnosing fetal abnormalities. B-FFE and SS-TSE can clearly show the anatomy of fetal organs and abnormal lesions; SS-TSE as a black blood sequence can be used to find the relationship between blood vessels and tumors; T1WI for the diagnosis of bleeding, fatty lesions have advantages; DWI to identify ischemia Hypoxic disease or arachnoid cyst is of great value. Therefore, in the fetus magnetic resonance imaging, four series of applications at the same time difficult to miss the lesion.
