设想脊髓损伤(SCI)平面以下自发性感觉异常性疼痛综合症(DPS)是由于SCI后躯体感觉系统的失神经传入所致的躯体感觉皮层重组所引起。故对16例有DPS的SCI病人,15例无疼痛的SCI病人和16例健康人的前臂、颈部和脊柱皮肤区做了两点分辨阈(2PDT)的检查。结果显示:疼痛组比非疼痛组及对照组有较低的2PDT,尤其是在脊柱和颈部。SCI疼痛组的脊柱皮肤区在DPS疼痛强度和2PDT之间呈明显的负相关,即疼痛强度越高,2PDT就越低。研究结果支持DPS起源于SCI后大脑躯体感觉皮层重组的设想。
It is hypothesized that spontaneous hyperalgesia syndrome (DPS) below the level of the spinal cord injury (SCI) is caused by somatosensory cortex recombination due to denervation of the somatosensory system after SCI. Therefore, 16 patients with DPS in SCI patients, 15 patients with painless SCI patients and 16 healthy subjects forearm, neck and spine skin area to do two-point discrimination threshold (2PDT) check. The results showed that the pain group had lower 2PDT than the non-pain group and the control group, especially in the spine and neck. Spinal skin area of SCI pain group showed a significant negative correlation between DPS pain intensity and 2PDT, that is, the higher the pain intensity, the lower the 2PDT. The findings support the idea that DPS originated from the somatic cortex remodeling of the brain after SCI.
