检验血流频谱分析在鉴别乳腺原发性肿块良恶性方面的价值并确立最佳的数值标准.方法:使用彩色多普勒血流显像(CDFI)和能量多普勒显像(PDI)分别引导脉冲多普勒(PWD)频谱分析,检查了104例乳腺原发性包块(良性55例63个病灶,恶性49例64个病灶),测量指标包括肿块周边与内部血流的收缩期峰速(PSV)、搏动指数(PI)及阻力指数(RI),井分别绘制受试者工作曲线确定各自的最佳阈值.结果:以CDFI状态取频谱,乳癌病灶周边及内部血流的平均PSV,PI和RI值均明显高于良性病变.而以PDI引导频谱分析,良恶性之间的差别较小.肿瘤周边血流PSV35.0cm/s,PI1.4和RI0.72是鉴别乳腺肿瘤良恶性的最佳阈值.结论:以彩色多普勒血流显像引导肿瘤血流频谱分析,对乳腺肿瘤良恶性的鉴别具有较高价值.
Test the value of blood flow spectrum analysis in the differentiation of benign and malignant breast primary tumors and establish the best numerical standard. Methods: Doppler flow imaging (CDFI) and power Doppler imaging (PDI) were used to guide pulsed-doppler (PWD) spectrum analysis. 104 primary breast masses (55 benign cases) were examined. 63 lesions, 49 malignant lesions (64 lesions). Measured parameters include systolic peak velocity (PSV), pulsatility index (PI), and resistance index (RI) around the mass and internal blood flow. Determine the optimal threshold for each. RESULTS: In the frequency spectrum of CDFI status, the average PSV, PI, and RI values of peripheral and internal blood flow of breast cancer lesions were significantly higher than those of benign lesions. With PDI guided spectrum analysis, the difference between good and bad is small. Peripheral blood flow PSV 35.0cm/s, PI1.4 and RI0.72 are the best thresholds for differentiating benign and malignant breast tumors. Conclusion: The analysis of blood flow spectrum guided by color Doppler flow imaging has a higher value for the differentiation of benign and malignant breast tumors.
