目的:观察慢性肾衰竭规律血液透析患者血清单个核白细胞肿瘤坏死因子-α(TNF-α)表达。方法:本研究包括健康对照组(Con组),慢性肾衰竭不伴冠心病组(CRF组)及慢性肾衰竭伴冠心病组(CRF+CHD组)。采用梯度离心法分离各组血清单核细胞并培养,制成细胞悬液,取培养的上清液,用放免法测定TNF-α。结果:(1)各组间例数、性别及年龄无差异,CRF组及CRF+CHD组伴高血压例数增多,明显高于对照组,CRF+CHD组患糖尿病例数明显高于对照组,p<0.05。(2)CRF组及CRF+CHD组血尿素氮及血肌酐高于Con组,CRF+CHD组FPG高于Con组,p<0.05。(3)CRF组及CRF+CHD组TNF-α浓度高于对照组,CRF+CHD组TNF-α浓度高于CRF组,p<0.05。结论:尿毒症合并冠心病患者外周血单个核白细胞分泌的TNF-α明显升高,分析与尿毒症患者的微炎症状态有关。
Objective: To observe the expression of tumor necrosis factor-α (TNF-α) in mononuclear leukocytes in hemodialysis patients with chronic renal failure. Methods: This study included healthy control group (Con group), chronic renal failure without coronary heart disease group (CRF group) and chronic renal failure with coronary heart disease group (CRF + CHD group). The serum mononuclear cells of each group were separated by gradient centrifugation and cultured to make cell suspension. The cultured supernatant was determined by radioimmunoassay. Results: (1) There was no difference in the number of cases, sex and age between the two groups. CRF and CRF + CHD patients had more cases of hypertension, which was significantly higher than that of control group. The number of diabetes patients in CRF + CHD group was significantly higher than that in control group , p <0.05. (2) The blood urea nitrogen and serum creatinine in CRF group and CRF + CHD group were higher than those in Con group. The FPG in CRF + CHD group was higher than that in Con group, p <0.05. (3) The concentration of TNF-α in CRF group and CRF + CHD group was higher than that in control group, the concentration of TNF-α in CRF + CHD group was higher than CRF group, p <0.05. Conclusion: TNF-α secreted by peripheral blood mononuclear leukocytes in patients with uremia complicated with coronary heart disease is significantly increased, and the analysis is related to the micro-inflammatory state of patients with uremia.
