目的:探讨前驱疱疹病毒感染对特发性血小板减少性紫癜(ITP)患者的细胞免疫功能及一线药物治疗效果的影响。方法:选取2016年1月—2017年1月期间收治的ITP患儿74例临床资料,根据有无前驱疱疹病毒感染,将其分为感染组和非感染组;分析两组患儿细胞免疫功能并经一线药物治疗后的血小板计数改善情况。结果:感染组患儿淋巴细胞亚群(THL、CD4/CD8)低于非感染组,而TSL高于非感染组(P<0.05);两组患儿的T细胞因子经组间比较其差异无统计学意义(P>0.05);感染组的IL-2、IL-4、IL-6、TNF等中位数低于非感染组,而IL-7有所增高,经一线药物治疗后,感染组患儿缓解率明显低于非感染组,经组间比较其差异无统计学意义(P>0.05)。结论:前驱疱疹病毒感染导致年幼儿ITP发病时免疫系统的TSL增多,除IL-7以外的Th细胞辅助因子均有下降趋势,但并不会影响患儿近期一线药物治疗效果。
Objective: To investigate the effects of herpes simplex virus infection on the cellular immune function and the first-line drug treatment in patients with idiopathic thrombocytopenic purpura (ITP). Methods: Totally 74 cases of ITP children admitted from January 2016 to January 2017 were selected and divided into infection group and non-infection group according to the presence or absence of herpes simplex virus infection. The cellular immune function And after first-line drug treatment of platelet count to improve the situation. Results: The levels of lymphocyte subsets (THL, CD4 / CD8) in infected children were lower than those in non-infected children and those in non-infected children (P <0.05). The differences of T-cell factor (P> 0.05). The median of IL-2, IL-4, IL-6 and TNF in infected group was lower than that in non-infected group, but IL-7 was increased. The response rate in infected group was significantly lower than that in non-infected group, and there was no significant difference between the two groups (P> 0.05). CONCLUSIONS: The infection of the herpes simplex virus leads to an increase of TSL in the immune system of young children with ITP onset. Th cofactors other than IL-7 have a decreasing trend, but they do not affect the recent first-line drug treatment in children.
