目的 :探讨鼻咽血管纤维瘤的 CT表现、手术效果、影响出血量和复发的因素。方法 :回顾分析了 39例鼻咽血管纤维瘤 ( NPAF)的临床表现、分期及手术方法。结果 :CT显示大多数 NPAF密度增高、边界清晰、并伴有骨质吸收 ;39例中复发 1 1例 ,复发率为 2 8.2 % ,平均复发时间为 6个月 ,肿瘤的复发与肿瘤分期有关 ( P<0 .0 5 ) ;术中出血量与患者的年龄、病程、肿瘤分期及手术进路的选择有关 ( P<0 .0 5 )。结论 :CT对 NPAF的诊断、范围确定及手术进路的选择具有重要的价值 ;复发的主要原因是手术中残留 ;硬腭进路具有暴露广泛、易寻找肿瘤根部、便于止血及面部不留切痕的优点 ,对于侵入翼腭窝内的肿瘤可采用硬腭 -齿槽后联合进路
Objective: To investigate the CT findings of nasopharyngeal angiofibroma, the effect of surgery, the factors affecting the amount of bleeding and relapse. Methods: A retrospective analysis of 39 cases of nasopharyngeal angiofibroma (NPAF) clinical manifestations, staging and surgical methods. Results: CT showed that the density of most NPAF was increased with clear border and bone resorption. Among the 39 cases, 11 cases were relapsed, the recurrence rate was 2.2% and the average recurrence time was 6 months. The recurrence of tumor was related to tumor stage (P0.05). The amount of bleeding during operation was related to the patient’s age, course of disease, tumor stage and surgical approach (P <0.05). Conclusion: CT is of great value in the diagnosis of NPAF, the determination of the scope and the choice of surgical approach. The main reason of recurrence is residual surgery. The extensive access to the palatal palate is easy to find the root of the tumor for hemostasis and no facial incision The advantages of invasive pterygopalatine tumors for the use of hard palate - coggy combined approach
