目的:了解糖耐量减低(IGT)人群颈动脉内膜中层厚度(IMT)与血清学指标相关性。方法:行标准75g葡萄糖耐量试验(OGTT),将108例IGT患者作为IGT组,120例糖耐量正常(NGT)体检者作为NGT组,采用彩色多普勒超声检查仪行颈动脉彩超检查,比较各组IMT水平;按IMT是否增厚将IGT组分为IMT正常组、增厚组,采用全自动生化分析仪测指标浓度;对各组年龄、血压、体质量指数(BMI)、血糖(FBG)、血清总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、血清尿酸(UA)、高敏C反应蛋白(CRP)、空腹血清胰岛素(FINS),计算稳态模型胰岛素抵抗指数(HOMA-IR)等临床资料与血清学指标进行比较分析。结果:IGT组FBG、2hPG、LDL-C、CRP、FINS、HOMA-IR、OGTT、IMT均高于NGT组;IMT增厚组平均年龄、BMI、2hPG、LDL-C、FINS、HOMA-IR、OGTT均高于IMT正常组,差别均有统计学意义,经多元回归分析IGT患者平均年龄、BMI、2hPG、LDL-C、FINS、HOMA-IR、OGTT均与IGT患者颈动脉IMT厚度相关。结论:平均年龄、BMI、2hPG、LDL-C、FINS、HOMA-IR、OGTT均是IGT人群IMT增厚的密切相关因素。
Objective: To investigate the relationship between carotid artery intima-media thickness (IMT) and serological parameters in patients with impaired glucose tolerance (IGT). Methods: The standard 75g glucose tolerance test (OGTT), 108 IGT patients as IGT group and 120 normal glucose tolerance group (NGT) as NGT group were examined by color Doppler ultrasound with color Doppler ultrasonography. The levels of IMT in each group were divided into IMT normal group and thickening group according to whether the IMT thickens or not, and the indexes of concentration are measured by automatic biochemical analyzer. The age, blood pressure, body mass index (BMI), blood glucose (FBG) (TC), triglyceride (TG), high density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C), serum uric acid (UA), high-sensitivity C-reactive protein ), Fasting serum insulin (FINS), and calculating the steady-state model of insulin resistance index (HOMA-IR) and other clinical data and serological indicators for comparative analysis. Results: The average age, BMI, 2hPG, LDL-C, FINS, HOMA-IR and the number of IMT in IGT group were significantly higher than those in NGT group (FBG, 2hPG, LDL-C, CRP, FINS, HOMA-IR, The mean age, BMI, 2hPG, LDL-C, FINS, HOMA-IR and OGTT of IGT patients were all correlated with the carotid IMT thickness in patients with IGT by using multiple regression analysis. CONCLUSION: Mean age, BMI, 2hPG, LDL-C, FINS, HOMA-IR and OGTT are all closely related factors of IMT thickening in IGT population.
