目的了解妊娠9~20周孕妇血清叶酸和同型半胱氨酸(Hcy)水平变化情况,为妊娠早期孕妇增补叶酸、预防妊娠并发症及胎儿出生缺陷提供参考。方法选取2015年1—5月在湖州市妇幼保健院早孕门诊建卡检查的单胎妊娠孕妇1 096人,184名健康育龄期非孕妇女作对照并对其进行1次血清FOL、Hcy检测。结果 1 096名妊娠9~20周孕妇血清FOL缺乏率6.75%,Hcy异常率1.46%;血清FOL平均值(13.03±4.79)ng/m L和Hcy平均值(5.78±1.68)μmol/L,分别与对照组的(9.29±3.71)ng/m L和(8.96±3.41)μmol/L比较,差异均有统计学意义(P=0.000);血清FOL水平在9~20孕周的孕妇中随着孕周增加逐渐下降(r=-0.220,P=0.000),在16~44岁的孕妇中随着年龄增长逐渐升高(r=0.222,P=0.000),血清Hcy水平在16~44岁的孕妇中随着年龄增长逐渐下降(r=-0.173,P=0.000),但与孕周未见相关性(r=0.035,P=0.251)。结论妊娠20周内孕妇血清叶酸营养状况较好,同型半胱氨酸异常率较低,建议根据不同孕周和年龄加强宣传教育。
Objective To understand the changes of serum levels of folic acid and homocysteine (Hcy) in pregnant women from 9 to 20 weeks of gestation, and to provide reference for the supplementation of folic acid in pregnant women in the first trimester, prevention of pregnancy complications and birth defects. Methods From January to May 2015, 1 096 singletonic pregnant women and 184 healthy women of child-bearing age were enrolled in this study. The levels of FOL and Hcy in serum were detected by ELISA in Huzhou MCH hospital. Results The serum FOL deficiency rate was 6.75% and the Hcy abnormality rate was 1.46% in 1 096 pregnant women with 9-20 weeks of gestation. The serum FOL average value was (13.03 ± 4.79) ng / m L and the mean Hcy was 5.78 ± 1.68 μmol / L Compared with control group (9.29 ± 3.71) ng / m L and (8.96 ± 3.41) μmol / L, the difference was statistically significant (P = 0.000); with serum FOL level in pregnant women with 9-20 gestational weeks The gestational age decreased gradually (r = -0.220, P = 0.000), and gradually increased with age in 16-44 years pregnant women (r = 0.222, P = 0.000) Pregnant women gradually decreased with age (r = -0.173, P = 0.000), but no correlation with gestational age (r = 0.035, P = 0.251). Conclusion Pregnant women have better serum folate status and lower homocysteine rate within 20 weeks of gestation. It is suggested that publicity and education should be strengthened according to different gestational ages and ages.
