胃泌素是刺激胃酸分泌的主要激素,循环中胃泌素浓度在十二指肠溃疡(DU)发病中的意义已被广泛研究。缩胆囊素(CCK)在结构上和胃泌素很相似,特别是具有生物活性的羧基端,二者羧基端5个氨基酸残基相同。CCK单独应用可轻度刺激胃液分泌,但CCK及其同类物可抑制由五肽胃泌素刺激所产生胃酸分泌的70%。因此CCK被认为作用于壁细胞的胃泌素的竞争性抑制剂。对于DU病人血浆CCK浓度及其释放了解甚少,本文在此方面作了研究。 22例DU病人,其中13例有活动症状,9例为静止期。诊断依据为上消化道对比造影或/和纤维胃十二指肠镜检。所有病人均无梗阻症状。6例活动性DU病人施行了迷走神经干切断和引流术。术后7~14天恢复正常饮食后复查。18例正常者作为对照。病员禁食12小时在口服Lipomul玉米油1.5ml/kg
Gastrin is the main hormone that stimulates gastric acid secretion. The significance of circulating gastrin concentration in the pathogenesis of duodenal ulcer (DU) has been extensively studied. Cholecystokinin (CCK) is structurally very similar to gastrin, especially with a biologically active carboxyl end, with the same five carboxy terminal amino acid residues. CCK alone can slightly stimulate gastric secretion, but CCK and its analogs inhibit 70% of the gastric acid secretion produced by pentagastrin stimulation. CCK is therefore considered a competitive inhibitor of gastrin in parietal cells. Little is known about the plasma CCK concentration and its release in DU patients, and this article has been studied in this regard. Twenty-two DU patients, of whom 13 had active symptoms and 9 were quiescent. Diagnosis is based on upper gastrointestinal contrast imaging or / and fiber gastroduodenoscopy. All patients had no symptoms of obstruction. Six patients with active DU underwent vagotomy and drainage. After 7 to 14 days after the return to normal diet review. 18 normal subjects as a control.
