铜针通电治疗血管瘤并发急性溶血的临床与实验研究

目的 探讨铜针通电治疗血管瘤并发急性血管内溶血的原因。方法 ①临床观察 1例巨大海绵状血管瘤两次铜针通电治疗均并发严重溶血的全过程。②体外实验分别将铜针、铁针和不锈钢针置于健康人枸橼酸抗凝血中 ,接 3~ 6V直流电 30min ,在通电前、中、后检测各标本血浆游离Hb值 ;推血片光镜下观察RBC形态。结果 临床病例两次铜针通电治疗均发生严重血管内溶血 ;体外实验通电后RBC变形破碎 ,血浆游离Hb明显升高。电压越高 ,通电时间越长 ,RBC破坏越严重。结论 铜针通电治疗巨大血管瘤可能并发严重的急性血管内溶血。电流对RBC的直接损伤是导致溶血的主要因素。 Objective To investigate the reasons for the acute intravascular hemolysis caused by electroacupuncture of copper needles in treating hemangiomas. Methods ① clinical observation of a case of giant cavernous hemangioma twice copper acupuncture treatment were complicated by severe hemolysis of the whole process. ② In vitro experiments, the copper needles, iron needles and stainless steel needles were placed in the anticoagulant of citric acid of healthy people, then the DC of 3 ~ 6V was connected for 30min, the plasma free Hb value of each specimen was detected before, during and after the electrification; Light microscope observation of RBC morphology. Results In clinical cases, severe intravascular hemolysis occurred after two doses of copper needle electrotherapy. The RBCs were broken and the plasma free Hb was significantly increased in vitro. The higher the voltage, the longer the power-on time, the more severe the RBC destruction. Conclusion Copper acupuncture treatment of giant hemangiomas may be complicated by severe acute intravascular hemolysis. Direct current RBC damage is the main factor leading to hemolysis.
