目的探讨吸烟对男性生殖激素水平的影响。方法于2013年8月至9月以社区(居委会或村)为单位,采用分层整群抽样方法抽取遵义地区20岁及以上成年男性1 213人进行问卷调查,并空腹抽取静脉血,检测生殖激素水平。按烟龄和吸烟量分为非吸烟组、吸烟A组(烟龄≤15年且日吸烟量<20支)、吸烟B组(烟龄≤15年且日吸烟量≥20支)、吸烟C组(烟龄>15年且日吸烟量<20支)、吸烟D组(烟龄>15年且日吸烟量≥20支),分析各组生殖激素水平。结果年龄与烟龄(r=0.330)、累积吸烟量(r=0.214)、性激素结合球蛋白(SHBG,r=0.458)、黄体生成素(LH,r=0.514)呈正相关;与游离睾酮(c FT,r=-0.429)、睾酮分泌指数(TSI,r=-0.463)、游离睾酮指数(FTI,r=-0.540)均呈负相关。以年龄为协变量进行协方差分析显示,吸烟C组人群血清TT水平高于非吸烟组、吸烟B组和吸烟D组,吸烟B组血清TT水平低于吸烟A组;吸烟C组、吸烟D组血清FTI水平均低于非吸烟组;吸烟A组血清TSI高于非吸烟组和吸烟D组,吸烟D组血清TSI水平低于吸烟C组,上述差异均有统计学意义(P<0.05)。结论少量短期吸烟可能会导致睾酮分泌能力应激性提高;而长期大量吸烟可能会导致睾酮分泌能力下降,活性睾酮水平降低。
Objective To investigate the effects of smoking on male reproductive hormones. Methods From August to September in 2013, community-based (neighborhood or village) units were stratified cluster sampling method to survey 1,131 adult males aged 20 years and over in Zunyi district for questionnaire survey. Blood samples were drawn from venous blood for reproductive testing Hormone levels. Group A (smoking ≤ 15 years and daily smoking <20 cigarettes), smoking group B (smoking ≤ 15 years and daily smoking ≥ 20 cigarettes), smoking C Group D (smoking> 15 years and daily smoking <20), smoking group D (smoking> 15 years and daily smoking ≥ 20), and analyzed the levels of reproductive hormones. Results There was a positive correlation between age and smoking age (r = 0.330), cumulative smoking (r = 0.214), sex hormone binding globulin (SHBG, r = 0.458) and luteinizing hormone (LH, r = 0.514) FT, r = -0.429), testosterone secretion index (TSI, r = -0.463) and free testosterone index (FTI, r = -0.540). Covariance analysis using age as a covariate showed that the serum TT level in smokers C group was higher than that in non-smoker group, smoking group B and smoking group D, smoking TT group was lower than smoker A group, smoking C group, smoking D The levels of serum TTI in serum of group A were higher than that of non-smoking group and smoking group D, while the serum TSI levels of smoking group D were lower than those of smoking group C (P <0.05) . Conclusion A small amount of short-term smoking may lead to increased stress testosterone secretion; long-term smoking may lead to testosterone secretion decreased activity testosterone levels decreased.
