目的:优化医院船对外医疗服务过程中手术流程,加强与病房之间密切协作,提高医院船手术室工作效率。方法:通过制定方案、预案,组织协调,人员调整和培训等措施,动态调整工作流程和人员结构,加快病房床位周转,提高日手术通过量。结果:不断完善手术组与病房组的工作流程,三站完成的手术种类分别是牙买加站10种,特立尼达和多巴哥站15种,哥斯达黎加站21种;参与手术科室牙买加站5个,特立尼达和多巴哥站7个,哥斯达黎加站7个;手术量分别为牙买加27台,特立尼达和多巴哥站34台,哥斯达黎加56台。就平均日手术量,牙买加站5.5台,特立尼达和多巴哥站6.4台,哥斯达黎加站11.2台;就单个手术台日通过量,牙买加站1.8台,特立尼达和多巴哥站2.1台,哥斯达黎加站3.7台。通过优化手术组与病房组的工作流程,病房日均床位周转次数逐步缩短,在哥斯达黎加医疗服务期间,日均床位周转次数为0.53例/(床·天),是牙买加医疗服务期间的1.9倍。同时,床位使用率也是逐步增加,哥斯达黎加站床位使用率为84.09%,较牙买加时提高40.34%。病房患者收治数与手术数量增加呈显著相关性,r=0.964。结论:手术组与病房组的密切协同对顺利、高效、优质完成医院船海外医疗服务任务起到了重要作用。
OBJECTIVE: To optimize the surgical procedure in the external medical service of hospital ships, strengthen the close cooperation with the wards and improve the operating efficiency of the operating room of hospital ships. Methods: Through the formulation of plans, plans, organization and coordination, personnel adjustment and training and other measures to dynamically adjust the workflow and personnel structure, to speed up ward bed turnover and improve the daily surgical throughput. Results: The workflow of the surgery group and the ward group was continuously improved. The types of surgery performed by the three stations were 10 from Jamaica, 15 from Trinidad and Tobago, and 21 from Costa Rica. Five involved in the surgical department of Jamaica Seven in Trinidad and Tobago and seven in Costa Rica; the number of operations was 27 in Jamaica, 34 in Trinidad and Tobago and 56 in Costa Rica. In terms of the average daily surgery volume, there were 5.5 Jamaica stations, 6.4 Trinidad and Tobago stations and 11.2 Costa Rica stations. For single operation table daily throughput, Jamaica station 1.8 units, Trinidad and Tobago station 2.1 units, Costa Rica station 3.7 units. By optimizing the workflow of the surgical and ward groups, the number of beds per bed per day was gradually reduced. During the Costa Rican medical service, the number of bed turns per day was 0.53 cases / (bed · day), 1.9 times that of Jamaica during the period of medical services. At the same time, the utilization rate of beds was also gradually increased. The utilization rate of beds in Costa Rica stood at 84.09%, an increase of 40.34% over that in Jamaica. There was a significant correlation between the number of patients admitted to the ward and the number of operations, r = 0.964. Conclusion: The close cooperation between the surgical group and ward plays an important role in the successful, high-quality and high-quality hospital overseas medical service.
