目的 :探讨尿五蛋白在顺铂 (DDP)化疗肾毒副作用早期诊断的临床意义。方法 :动态检测 72例患者接受 DDP(6 0 m g/ m2~ 80 mg/ m2 )化疗 72个周期的尿 β2 -微球蛋白 (β2 - MG)、α1 -微球蛋白 (α1 - MG)、白蛋白(Alb)、转铁蛋白 (TRF)、免疫球蛋白 G(IGG) ,以及血清尿素氮 (BU N) ,肌酐 (Cr) ,筛选出有价值的早期诊断顺铂肾毒副作用的指标。结果 :化疗早期 (d1 ,d3,d5 ) ,患者 β2 - MG和 α1 - MG出现异常增高的周期数明显多于其他指标 (P<0 .0 0 1) ,且患者尿 α1 - MG数又明显多于尿 β2 - MG(P<0 .0 0 1) ,而血清 BU N、Cr异常增高的周期数为 0。结论 :β2 -MG和 α1 - MG可作为早期检测顺铂化疗肾毒副作用的指标 ,α1 - MG优于 β2 - MG。
Objective: To explore the clinical significance of urinary pentaprotein in the early diagnosis of nephrotoxic side effects of cisplatin (DDP) chemotherapy. METHODS: A total of 72 patients undergoing 72 cycles of DDP (60 mg/m2 to 80 mg/m2) chemotherapy were dynamically tested for urinary β2-microglobulin (β2-MG), α1-microglobulin (α1-MG), and white blood. Proteins (Alb), transferrin (TRF), immunoglobulin G (IGG), and serum urea nitrogen (BU N) and creatinine (Cr) were screened for valuable early indicators of cisplatin nephrotoxicity. Results: In the early stage of chemotherapy (d1, d3, d5), the number of abnormal increase in β2-MG and α1-MG in patients was significantly more than other indicators (P<0.01), and the number of urine α1-MG in patients was significant. More than urine β2-MG (P<0.01), but the abnormal increase of serum N-N and Cr cycles was 0. Conclusion: β2-MG and α1-MG can be used as indicators of early detection of nephrotoxic side effects of cisplatin chemotherapy. α1-MG is better than β2-MG.
