目的探讨新生儿缺氧缺血性脑病在CT检查中的征象分析。方法收集我院近4年来35例新生儿缺氧缺血性脑病的CT资料,结合文献,分析新生儿缺氧缺血性脑病的CT征象。15例1叶脑组织小片状低密度影;13例2叶脑组织散在片状低密度影;7例灰白质界限不清,脑室受压变小,并伴有颅内出血(包括脑室内及室管膜下出血灶,蛛网膜下腔出血,硬膜下出血及脑内血肿)。16例为出生后1d,14例为出生后3d,5例为出生后4d。结果 CT平扫加薄扫即可发现新生儿缺氧缺血性脑病;CT分度为:15例为轻度,13例为中度,7例为重度新生儿缺氧缺血性脑病。结论 CT平扫加薄扫对新生儿缺氧缺血性脑病具有很高的敏感性,结合临床分期,明确其与CT分度之间的关系,预后判断,为临床治疗提供依据。
Objective To investigate the signs of neonatal hypoxic-ischemic encephalopathy in CT examination. Methods CT data of 35 neonates with hypoxic-ischemic encephalopathy collected in our hospital in the past 4 years were collected. According to the literature, CT signs of neonatal hypoxic-ischemic encephalopathy were analyzed. 15 cases of 1 leaflet cerebellar low density film; 13 cases of 2 leaf lobe scattered in low density film; 7 cases of gray matter boundaries is unclear, ventricular compression smaller, and associated with intracranial hemorrhage (including intraventricular and Subependymal hemorrhage, subarachnoid hemorrhage, subdural hemorrhage and intracerebral hematoma). 16 cases were 1 day after birth, 14 cases were 3 days after birth, and 5 cases were 4 days after birth. Results CT scan and thinning scan can be found in neonates with hypoxic-ischemic encephalopathy; CT index: 15 cases were mild, 13 cases were moderate, 7 cases of severe neonatal hypoxic-ischemic encephalopathy. Conclusions CT scan and thinning scan are highly sensitive to neonatal hypoxic-ischemic encephalopathy. Combined with clinical stage, the relationship between CT scan and CT score and prognosis are analyzed, which provide the basis for clinical treatment.
