目的探讨不同年龄层次的糖尿病患者糖化血红蛋白(Hb A1c)诊断标准。方法选择210例受检者,以60岁为分层分别计算<60岁受检者以及≥60岁受检者的Hb A1c诊断切点。结果 1 210例受检者尤登指数最大点所对应的Hb A1c切点为6.2%,其诊断敏感性为71.5%,特异度为74.9%;2 Hb A1c≥6.2%患糖尿病率为75.38%;Hb A1c<6.2%患糖尿病率为31.03%,差异有统计学意义(P<0.01);3年龄≥60岁的受检者尤登指数最大点所对应的Hb A1c切点为6.3%,年龄<60岁的受检者尤登指数最大点所对应的Hb A1c切点为6.0%;4在≥60岁的受检者中,以切点6.3%作为诊断切点对糖尿病诊断率明显高于以6.2%为诊断点的诊断率(P<0.05);在<60岁的受检者中,以切点为6.0%作为诊断切点对糖尿病诊断率明显高于其它两个诊断切点(P<0.05)。结论不同年龄层次Hb A1c诊断切入点不同,≥60岁诊断切入点为6.3%,<60岁诊断切入点为6.0%。
Objective To investigate the diagnostic criteria of Hb A1c in diabetic patients of different ages. Methods A total of 210 subjects were enrolled in this study. Hb A1c diagnostic cut-off points were calculated in patients <60 years old and in subjects ≥60 years of age at the age of 60 years. Results The Hb A1c cutoff point corresponding to the maximum of the Youden index in 1 210 cases was 6.2%, the diagnostic sensitivity was 71.5% and the specificity was 74.9%. The prevalence of diabetes was 75.38% in 2 Hb A1c≥6.2% The prevalence of Hb A1c was 6.23%, the difference was statistically significant (P <0.01). The mean Hb A1c cutoff point of Youden’s index was 6.3% The Hb A1c cut-off point corresponding to the maximum point of the Youden index of 60-year-old subjects was 6.0%. 4 Among the test subjects ≥60 years of age, the diagnosis rate of diabetes was 6.3% The diagnosis rate of diabetes was 6.2% (P <0.05). Among the subjects under 60 years of age, the diagnosis rate of diabetes was significantly higher than that of the other two diagnostic cut points (6.0% 0.05). Conclusions The diagnosis of Hb A1c is different at different age groups. The diagnosis point of incidence of ≥60 years old is 6.3%, and the diagnosis point of <60 years old is 6.0%.
