目的:探讨中药黄龙汤对脓毒症大鼠免疫功能的保护作用及机制。方法:将55只Wistar大鼠按随机数字表法分为假手术组(10只)、模型组(15只)、西药治疗组(15只)和中西药联合治疗组(15只)。采用盲肠结扎穿孔术(CLP)复制脓毒症模型,假手术组只手术,不行CLP。假手术组和模型组灌胃2 m L生理盐水;西药对照组腹腔注射美罗培南每次50 mg;中西药联合治疗组在西药治疗基础上灌胃中药黄龙汤[生大黄(后下)15 g,芒硝10 g,枳实15 g,厚朴10 g,人参10 g,当归20 g,黄芩15 g,甘草10 g,大枣12 g,煎浓成150 m L]每次2 m L(含生药0.68 g/m L),4组均每日2次。给药4 d后处死大鼠,腹主动脉取血。运用流式细胞仪检测T淋巴细胞亚群CD4、CD8的变化。结果:本研究结果显示,西药组、中药+西药组回肠黏膜淋巴细胞中CD_4~+、CD_8~+T细胞及CD_4~+T/CD_8~+T值均较模型对照组显著上升(P<0.05)。中药+西药组CD_4~+T细胞较西药组明显升高(P<0.05)。结论:中药汤剂黄龙汤在脓毒症大鼠中具有提高CD_4~+T细胞计数,对抗脓毒症免疫抑制,提高脓毒症模型大鼠生存率的作用。
Objective: To investigate the protective effect and mechanism of traditional Chinese medicine Huanglong Decoction on immune function in septic rats. Methods: Fifty-five Wistar rats were randomly divided into sham operation group (n = 10), model group (n = 15), western medicine group (n = 15) and combination of western medicine group (n = 15). Cecal ligation and puncture (CLP) was used to replicate the sepsis model. Sham-operated group received only surgery without CLP. Sham-operation group and model group were given 2 ml normal saline. The western medicine control group was given intraperitoneal injection of meropenem 50 mg each time. The combination of traditional Chinese medicine and western medicine on the basis of Western medicine was given Chinese medicine Huanglong Decoction [rhubarb , Glauber’s salt 10 g, Fructus aurantii 15 g, Magnolia bark 10 g, ginseng 10 g, Angelica sinensis 20 g, Scutellaria baicalensis 15 g, licorice 10 g, jujube 12 g, decoction 150 m L] Crude drug 0.68 g / m L), 4 groups were twice daily. Rats were sacrificed 4 days after dosing, abdominal aorta blood. The changes of CD4 and CD8 in T lymphocyte subsets were detected by flow cytometry. Results: The results showed that CD_4 ~ +, CD_8 ~ + T cells and CD_4 ~ + T / CD_8 ~ + T of ileum mucosal lymphocytes in western medicine group, traditional Chinese medicine and western medicine group were significantly increased compared with model control group (P <0.05 ). Compared with western medicine group, CD_4 ~ + T cells in traditional Chinese medicine + western medicine group were significantly increased (P <0.05). Conclusion: Decoction of decoction of Huanglong decoction has the function of increasing CD_4 ~ + T cell count, antagonizing sepsis immunosuppression and improving the survival rate of sepsis model rats.
