目的:分析我院2015年度门诊处方点评情况,为提高处方书写规范性和合理用药水平提供参考。方法:依据《处方管理办法》《医院处方点评管理规范(试行)》,以及药品说明书、《新编药物学》等文献资料,采用Excel表,对2015年度门诊处方32 400张书写的规范性及药物临床使用的适宜性进行点评与统计。结果:在32 400张处方中,发现不合格处方2021张,占6.2%。在2021张不合格处方中,不规范处方856张,占42.4%;用药不适宜的处方1064张,占52.6%;超常处方101张,占5.0%。结论:我院门诊处方中不合格处方占一定比例,应采取相应措施加以改进。
OBJECTIVE: To analyze the review of outpatient prescriptions in 2015 in our hospital and provide references for improving the normative and rational drug use of prescriptions. Methods: Based on the “prescription management approach”, “hospital prescription reviews management practices (for trial implementation)”, and drug documentation, “new pharmacology” and other documents, the use of Excel tables, 2015 outpatient prescription 32 400 norms and The suitability of clinical use of drugs reviews and statistics. Results: Of the 32 400 prescriptions, 2021 failed prescriptions were found, accounting for 6.2%. Among 2021 unqualified prescriptions, 856 were non-standard prescriptions, accounting for 42.4%; 1064 prescriptions inappropriate for medication, accounting for 52.6%; and 101 prescriptions exceeding prescriptions, accounting for 5.0%. Conclusion: In our hospital outpatient prescriptions in a certain proportion of substandard prescriptions, should take appropriate measures to be improved.
