目的观察早产儿视网膜病变(ROP)的彩色多普勒血流成像(CDFI)检查的形态特点和血流特征,为ROP临床诊断提供新的依据。方法对78例有典型早产、出生体重低等病史并经散瞳双目间接检眼镜眼底检查确诊的4、5期ROP患儿的156只患眼进行CDFI检查,观察其CDFI形态特点及病变的血流特征。结果4期病例主要表现为玻璃体内有起自一侧周边眼球壁的弱条带状回声,向后极部与眼球壁回声相延续并与视盘回声相连;5期病例主要表现为与晶状体回声紧密相连并包绕其周围的团状回声,合并视网膜脱离时病变类似荷花状,前段膨大的“花体”包绕晶状体并与其紧密相连,向后逐渐变细为“茎部”,呈弱条带状回声与视盘相连。血流特点表现为病变的“茎部”可见与视网膜中央动静脉相延续的血流信号,脉冲多普勒频谱分析为动脉静脉伴行的血流频谱,与视网膜中央动脉静脉完全相同。结论4、5期ROP患儿CDFI主要表现为玻璃体内带状或团状回声,其中可见与视网膜中央动静脉相延续的血流信号。根据CDFI形态改变和血流特征有助于ROP的诊断。
Objective To observe the morphological characteristics and blood flow characteristics of color Doppler flow imaging (CDFI) in retinopathy of prematurity (ROP) and provide a new basis for the clinical diagnosis of ROP. Methods CDFI was performed in 156 eyes of 45 ROP children with stage 4 and 5 diagnosed by funduscopic examination of mydriasis binocular ophthalmoscope in 78 patients with typical preterm birth and low birth weight. Flow characteristics of the lesion showed “stem” visible with the central retinal artery and vein phase of the continuation of the blood flow signal, pulse Doppler spectral analysis of arterial venous accompanying blood flow spectrum, and the central retinal artery vein exactly the same. Conclusions In stage 4 and 5, CDFI in ROP children mainly showed band-like or group-like echo in the vitreous, and the blood flow signal was found to be continuous with central retinal artery and vein. According to CDFI morphological changes and blood flow characteristics contribute to the diagnosis of ROP.
