目的:探讨老年原发性高血压患者静息心率与靶器官损害的关系。方法:入选老年原发性高血压患者619例,按静息心率水平分为3组。对入选患者做以下检查:心电图、超声心动图、颈动脉超声、肌酐清除率以及尿微量白蛋白及眼底动脉造影。结果:B组和C组的颈动脉内膜厚度、颈动脉内径、左心室质量指数以及尿微量白蛋白定量均高于A组(P<0.01或P<0.05),而左室射血分数、肌酐清除率小于A组(P<0.01或P<0.05),B及C组较A组眼底动脉硬化程度重(P<0.01或P<0.05)。结论:静息心率增加可能是老年原发性高血压患者靶器官损害的独立危险因素。
Objective: To investigate the relationship between resting heart rate and target organ damage in elderly patients with essential hypertension. Methods: 619 elderly patients with essential hypertension were enrolled and divided into 3 groups according to resting heart rate. The following patients were selected for examination: electrocardiogram, echocardiography, carotid ultrasound, creatinine clearance and urinary albumin and fundus arteriography. Results: The carotid intima-media thickness, carotid artery diameter, left ventricular mass index and urinary microalbuminuria in group B and group C were higher than those in group A (P <0.01 or P <0.05), while left ventricular ejection fraction, Creatinine clearance rate was lower in group A than in group A (P <0.01 or P <0.05). Group B and C were more severe than those in group A (P <0.01 or P <0.05). Conclusion: Increased resting heart rate may be an independent risk factor for target organ damage in elderly patients with essential hypertension.
