Cholesteatoma direct invasion of the damage caused by the loss has been considered a serious threat to the inner ear function. The authors report that 12 cases of lost patients were extensively damaged and their preoperative hearing was good. The results of preoperative CT and intraoperative confirmed that in 12 cases, 5 cases of middle ear cholesteatoma, 7 cases have been infiltrated and temporal bone, lost at least involving two semicircular canal. Five cases had pathological changes of all semicircular canal. Six cases had vestibular involvement. One case of cholesteatoma exactly eroded three semicircular canal, vestibular and endolymphatic vessels and oval capsule. Two cases of cholesteatoma affected cochlear basal turn and upper and lower semicircular canal or posterior Semi-regulatory. Preoperative and postoperative masking of the bone conduction values for the frequency 0.25,0.5,1,2 KHz hearing threshold average, 12 cases of preoperative bone conduction value between 5dB ~ 22dB (average 16dB), postoperative 5 cases of cochlear function
