目的: 探讨不同体重指数(Body Mass Index, BMI)下肺部个性化低剂量CT扫描方案在体检筛查及降低辐射剂量中的作用.材料和方法:随机将2017年8月-12月健康体检者分成两组,A组CT检查管电压固定100KV,管电流视患者BMI不同,给予相应的扫描方案(低体重指数组:BMI<20, 20mA,中体重指数组:20≤BMI≤25,30mA;高体重指数组:BMI>25, 40mA);B组行常规低剂量肺部CT检查,管电压固定100KV,管电流固定40mA,对比2组患者的有效辐射剂量、图像质量及结节检出率.结果:所有体检图像上观评分均>3分,满足诊断要求;A组图像主观评分为4.6±0.7;B组图像主观评分4.5±0.5,2组图像主观评分差异无统计学意义(P>0.05),A组有效剂量0.47±0.07msv,B组有效剂量0.52±0.09msv,A组有效剂量明显低于B组 (P<0.05).A组有34例发现阳性结节(34/157,21.2%),其中老年组20例,吸烟组22例;B组有37例存在阳性结节(37/162,22.8%),其中老年组23例,吸烟组27例;2组患者在结节检出率及结节检出分布情况无明显统计学差异(P>0.05).结论:以BMI为基础的个性化肺部低刑量CT扫描在肺部结节筛查中能降低辐射剂量,且不影响图像质量.“,”Objective: To explore the effect of personalization low-dose muhislice spiral CT scanning based on BMI in Medical screening. Methods: Healthy controls were involved this study from April to June 2016, which were divided into two groups randomly. In A group, the patients were performed 100KV fixed tube voltage, personalization tube current according to different BMI(low BMI group: BMI < 20, 20 mA; intermedlate BMI group: 20≤BMI≤25, 30mA; high BMI group: BMI>25, 40mA). In B group, the patients were performed conventional low-dose muhislice spiral CT scanning fixed 100KV fixed tube voltage and 40 mA tubes current. The effective dose, Image quality and nodule detection rate were compared between two groups. Results: The subjeetive scores of image quallty were > 3 point, which all met the dlagnostic need. The Image quality subjective scoring were 4.6±0.7, compared to 4.5±0.5 in B group, there was were not statistically significant between two group (P>0.05). The effective dose in A group was 0.47±0.07msv, compared to 0.52±0.09msv in B group, there were was Statistically significant between two group (P<0.05) ; 34 positive nodules (34/157,21.2%) were detected in group A, in which 20 cases were in the elderly group and 22 in the smoking group. 37 positive nodules in group B (37/162,22.8%)Were detected in group B, in which 23 cases were in the elderly group and 27 were in the smoking group. There was no statistically significant difference between two groups (P<0.05). Condusion: Personalization low-dose muhislice spiral CT scanning based on BMI in Medical screening not only reduce effective dose, but also guaranteeing image quality.
