红脖游蛇咬伤中毒的临床研究报告

目的 :为了探明红脖游蛇 (Rhabdophis subminiatus)咬伤中毒引起出血的机理。方法 :采取临床观察结合常规检查血小板计数 (PL T)及试管法凝血时间 (CT)等实验室捡查 ,其中 3例进行了抗凝血酶原 活性 (AT- )、α2 -纤溶酶抑制物活性 (α2 -PI)、纤维蛋白原浓度 (Fg)和纤维蛋白降解产物含量 (FDP)测定的血液学研究。结果 :所有的病人 PL T均在正常范围 ,但 CT明显延长 ,3例病人的 AT- 、α2 - PI活性明显下降 ,其中α2 - PI为甚 ;纤维蛋白浓度 (Fg)明显被消耗 ,纤维蛋白降解产物含量(FDP)明显增多。结论 :红脖游蛇咬伤可引起患者体内凝血系统被激活而继发纤溶亢进完全脱纤维蛋白状态的类 DIC样血液学改变。 Objective: To investigate the mechanism of bleeding caused by bites of Rhabdophis subminiatus. Methods: The clinical observation combined with routine examination of platelet count (PL T) and test tube clotting time (CT) laboratory tests, including 3 cases of antithrombin activity (AT-), α2 - plasmin inhibition Hematology studies were performed to determine the activity (α2 -PI), fibrinogen concentration (Fg) and fibrinolytic product content (FDP). Results: The PLT of all the patients was in the normal range, but the CT was prolonged. The activity of AT - and α2 - PI in 3 patients was significantly decreased. The α2 - PI was even higher. The fibrin concentration (Fg) Degradation product content (FDP) increased significantly. CONCLUSIONS: Red-necked snake bite can cause DIC-like hematologic changes in patients with coagulation system activation and complete fibrinolysis secondary to fibrinolysis.
