目的探讨潜伏期分娩镇痛对孕妇妊娠结局和新生儿预后的影响。方法选取2013年5月~2014年3月间在该院产科住院分娩的单胎初产妇186例,随机分为观察组和对照组,每组各93例。观察组产妇给予潜伏期椎管内阻滞麻醉分娩镇痛;对照组给予第一产程活跃期分娩镇痛。比较两组产妇的产程时间、疼痛感、分娩结局、围产儿结局。结果观察组产妇的第一产程和第二产程的时间均明显短于对照组,差异有统计学意义(P<0.05);观察组产妇第一产程开始后5 min、30 min、60 min的视觉模拟评分(VAS评分)明显低于对照组,差异均有统计学意义(P<0.05)。观察组产妇顺产率和阴道分娩率均明显高于对照组,差异有统计学意义(P<0.05);观察组产妇剖宫产率明显低于对照组,差异有统计学意义(P<0.01)。观察组新生儿窘迫率明显低于对照组,新生儿血糖水平、Apgar评分明显高于对照组,差异均有统计学意义(P<0.05)。结论潜伏期行椎管内阻滞麻醉分娩镇痛可有效缩短产程时间、减轻分娩疼痛,有助于提升产妇自然分娩信心,降低剖宫产率,有效改善母婴结局。
Objective To investigate the effect of latent labor analgesia on pregnancy outcomes and the prognosis of newborns. Methods A total of 186 singleton primipara from January 2013 to March 2014 in our hospital were randomly divided into observation group and control group, with 93 cases in each group. Observation group maternal administration of anesthesia for latent spinal anesthesia labor analgesia; the control group given labor during the active labor analgesia. The duration of labor, pain, delivery outcome and perinatal outcome were compared between the two groups. Results The first and second stage of labor in the observation group were significantly shorter than those in the control group (P <0.05). The visual acuity of the observation group at 5 min, 30 min and 60 min after the start of the first stage of labor The simulated score (VAS score) was significantly lower than that of the control group, the difference was statistically significant (P <0.05). The observation group maternal cistern rate and vaginal delivery rate were significantly higher than the control group, the difference was statistically significant (P <0.05); observation group maternal cesarean section rate was significantly lower than the control group, the difference was statistically significant (P <0.01) . The neonatal distress rate in the observation group was significantly lower than that in the control group. The neonatal blood glucose level and Apgar score were significantly higher than those in the control group (P <0.05). Conclusion The incubation period of spinal canal anesthesia anesthesia labor analgesia can shorten the labor time and reduce labor pain, help to enhance the confidence of maternal natural childbirth, reduce the rate of cesarean section and effectively improve the maternal and infant outcomes.
