目的:探讨慢性肾功能衰竭(CRF)患者血清高分子量脂联素多聚体(HMW-APN)水平及临床意义。方法:ELISA方法测定55例非血液透析的CRF患者(A组),58例血液透析的CRF患者(B组)和50例健康人(C组)血清HMW-APN、总APN水平、肾功能和血脂,并测定颈动脉内膜-中膜厚度(IMT)及粥样斑块发生率,分析各指标间的相关性。结果:1.A和B组血清HMW-APN、总APN水平等均高于C组,B组IMT厚度和粥样斑块发生率均高于A组(P<0.05),B组血清HMW-APN水平低于A组(P<0.05);2.HMW-APN水平与甘油三酯(TG)呈负相关(r=-0.312,P<0.05),与尿素氮(BUN)、肌酐(Cr)呈正相关(r=0.713、0.512,P<0.05);总APN水平与肌酐(Cr)呈正相关(r=0.681,P<0.05)。结论:同时检测HMW-APN和总APN的水平是观察CRF的一个较好的指标。
Objective: To investigate the serum level of adiponectin (HMW-APN) and its clinical significance in patients with chronic renal failure (CRF). Methods: Serum levels of HMW-APN, total APN, renal function, serum creatinine and total cholesterol in 55 non-hemodialysis CRF patients (group A), 58 hemodialysis CRF patients (group B) and 50 healthy people Blood lipids, carotid artery intima-media thickness (IMT) and the incidence of atherosclerotic plaque were measured, and the correlation between the indexes was analyzed. The serum levels of HMW-APN and total APN in group A and group B were higher than those in group C, while the IMT thickness and the incidence of atheroma in group B were significantly higher than those in group A (P <0.05) APN levels were lower than those in group A (P <0.05); 2.HMW-APN levels were negatively correlated with triglyceride (r = -0.312, P < (R = 0.713,0.512, P <0.05). There was a positive correlation between total APN level and creatinine (r = 0.681, P <0.05). Conclusion: Simultaneous detection of HMW-APN and total APN levels is a good indicator of CRF.
