目的:应用时间-空间相关成像技术(STIC)定量评估中孕期正常胎儿左心室功能。方法:对150例孕21~32周6天正常产检孕妇应用STIC技术扫描获得胎儿心脏整个心动周期的容积数据,存盘后采用QLAB软件进行脱机分析,左心室舒张期将三维图像切片成多个二维图像,手工勾勒左心室内缘,软件自动三维重建,并计算出左心室舒张末期容积(LEDV),同样方法得出左心室收缩末期容积(LESV),并计算出左心室每搏输出量(LSV),分析不同孕龄的胎儿左心室这3个参数与孕龄的关系。结果:150例检查中只有135例获得满意图像进行分析。LEDV、LESV及LSV随孕龄增加而增加,左室舒张末期容积与孕龄(X)关系公式:LEDV(ml)=-3.85+0.2X(R2=0.977,P<0.05);左室收缩末期容积与孕龄关系公式:LESV(ml)=-1.282+0.066X(R2=0.954,P<0.05);左心室每搏输出量与孕龄关系:LSV(ml)=-2.568+0.133X(R2=0.978,P<0.05)。结论:STIC技术能较为精确地计算胎儿心脏容积,并能用于临床评估胎儿心脏功能。
OBJECTIVE: To quantitatively assess normal fetal left ventricular function during pregnancy using time-space-related imaging (STIC). Methods: 150 pregnant women with gestational age from 21 to 32 weeks and 6 days normal pregnant women were scanned with STIC technique to get the volume data of the entire cardiac cycle of fetal heart. After storage, QLAB software was used for offline analysis. Left ventricular diastolic phase was sliced into multiple Two-dimensional images were manually sketched into the left ventricle and the software was automatically reconstructed by three-dimensional reconstruction. The left ventricular end-diastolic volume (LEDV) was calculated. The left ventricular end-systolic volume (LESV) was also calculated and the left ventricular stroke volume (LSV), analysis of fetal gestational age of the left ventricle of these three parameters and gestational age. Results: Only 135 of the 150 cases obtained satisfactory images for analysis. LEDV, LESV and LSV increased with the increase of gestational age, the relationship between left ventricular end diastolic volume and gestational age (X): LEDV (ml) = -3.85 + 0.2X (R2 = 0.977, The relationship between volume and gestational age: LESV (ml) = - 1.282 + 0.066X (R2 = 0.954, P <0.05); The relationship between left ventricular stroke volume and gestational age: LSV (ml) = -2.568 + 0.133X (R2 = 0.978, P <0.05). Conclusion: The STIC technique can calculate the fetal heart volume more accurately and can be used for clinical evaluation of fetal heart function.
