目的探讨应用实时三维超声(RT-3DE)评价室间隔缺损(VSD)患者修补术前及术后不同时期室间隔功能变化的临床价值。方法行VSD修补术的患儿40例,于术前1周内、术后1周及术后6个月行RT-3DE检查,检测左室射血分数(LVEF)、校正的左室16节段达最小收缩末容积时间的标准差(Tmsv 16-SD%)及最大差(Tmsv 16-Dif%)、室间隔局部功能(rEF)、室间隔达最小收缩末容积的时间(T)。33例健康儿童作为对照组。结果术前VSD患者Tmsv 16-SD%、Tmsv 16-Dif%、rEF、T均较对照组增大(P<0.05或0.01);术后1周时LVEF、rEF明显减低(P<0.01),但Tmsv 16-SD%、Tmsv 16-Dif%和T与对照组比较无明显变化(P>0.05);术后6个月时rEF仍减低,其余参数均恢复至正常(P>0.05)。结论室间隔修补术后6个月室间隔局部收缩功能仍减低;对室间隔修补术后患者定期随访具有重要的临床意义。
Objective To evaluate the clinical value of real-time three-dimensional echocardiography (RT-3DE) in evaluating ventricular septal function in patients with ventricular septal defect (VSD) before and after repair. Methods Forty children with VSD repair were examined by RT-3DE within 1 week before operation, 1 week after operation and 6 months after operation. Left ventricular ejection fraction (LVEF), corrected left ventricular The standard deviation (Tmsv 16-SD%) and maximum difference (Tmsv 16-Dif%), the regional function of the interventricular septum (rEF), and the minimum (T) end-systolic volume of the ventricular end- 33 healthy children served as the control group. Results TMSv 16-SD%, Tmsv 16-Dif%, rEF and T were significantly increased (P <0.05 or 0.01) in patients with preoperative VSD compared with the control group. LVEF and rEF were significantly decreased at 1 week after operation (P <0.01) However, the Tmsv 16-SD%, Tmsv 16-Dif% and T had no significant difference compared with the control group (P> 0.05). At 6 months after operation, the rEF still decreased and the rest parameters returned to normal (P> 0.05). CONCLUSION: The local systolic function of ventricular septum still decreases after 6 months of interventricular septum repair. It is of important clinical significance to follow-up patients after septal septal repair.
