男,60岁,于轻劳动时突然昏倒,短暂意识不清,四肢抽搐,约30秒钟后意识恢复,伴胸闷、心悸,气促,头昏。即入院当时ECG示窦性心动过速(120次/分),Ⅲ度AVB,室性逸搏心律,室率30次/分。两年前确诊冠心病。偶有心绞痛发作。无晕厥史。体查:平卧,体温37℃,呼吸22次,脉搏42次,血压60mmHg。心界略向左扩大,心率42次,律不齐,心音较低钝,偶可闻及大炮音,无病理
