目的探讨CT血管造影(CT angiography,CTA)对先天性主动脉弓畸形的诊断价值,总结先天性主动脉弓畸形的CT表现。方法收集2008年7月—2015年7月在郑州大学第三附属医院行CT血管造影检查的先天性主动脉弓畸形病例156例(98例经手术证实,58例行DSA检查),所有病例在CTA检查前常规行心脏超声检查,并将CTA和超声检查结果与手术和DSA结果进行对比。结果 156例中,动脉导管未闭56例,主动脉缩窄32例,主动脉弓中断11例,永存第五对主动脉弓1例,双主动脉弓6例,非梗阻性其它主动脉弓畸形共50例。CTA的诊断准确率为98.7%(154/156),心脏超声检查的诊断准确率为87.2%(136/156)。结论先天性主动脉弓畸形表现多样,CTA是可靠的影像检查方法,为临床提供更丰富可靠的诊断信息。
Objective To investigate the diagnostic value of CT angiography (CTA) for congenital aortic arch deformity and to summarize the CT findings of congenital aortic arch deformity. Methods A total of 156 cases of congenital aortic arch deformity underwent CT angiography at the Third Affiliated Hospital of Zhengzhou University from July 2008 to July 2015 were enrolled in this study. Of the 98 cases confirmed by surgery, 58 cases underwent DSA examination. All patients underwent CTA examination Conventional cardiac echocardiography was routinely performed and CTA and ultrasound findings were compared with those of surgery and DSA. Results Among the 156 cases, 56 cases were patent ductus arteriosus, 32 cases were aortic constriction, 11 cases were aortic arch interruption, 5 cases were permanent aortic arch, 6 cases were bilateral aortic arch and 50 cases were non-obstructive aortic arch deformity. The diagnostic accuracy rate of CTA was 98.7% (154/156), and the diagnostic accuracy rate of echocardiography was 87.2% (136/156). Conclusions Congenital aortic arch deformity is diverse, and CTA is a reliable method for imaging examination, providing more reliable and reliable diagnostic information for clinic.
