目的:探讨颅内动静脉畸形(AVM)显微外科手术治疗的方法和效果,以提高脑动脉畸形的全切率,降低致残率和死亡率。方法:回顾分析我院2008年5月~2010年5月收治的23例AVM的临床资料。23例病例均在显微镜下全切,切除病灶15例,清除血肿同时切除病灶4例,4例行术前栓塞,然后行手术切除病灶。结果:术后立即病理检查证实AVM,无手术死亡。出院时疗效优良率20例(87%),病残3例(13%)。全部病例随访时间6~18个月,部分残留复发4例,其余均正常工作或生活自理。结论:显微外科是目前治疗AVM的主要方法,能够提高AVM的治愈率,减少并发症,降低手术风险,安全有效。
Objective: To explore the method and effect of intracranial arteriovenous malformation (AVM) microsurgery in order to improve the rate of cerebral arterial malformations and reduce the morbidity and mortality. Methods: The clinical data of 23 patients with AVM admitted to our hospital from May 2008 to May 2010 were retrospectively analyzed. Twenty-three cases underwent total resection under microscope. Fifteen cases were resected, 4 cases were resected with hematoma removal, 4 cases were treated with preoperative embolization, and then surgery was performed to remove the lesion. Results: Immediate postoperative pathology confirmed AVM, no operative death. Efficacy at discharge rate of 20 cases (87%), 3 cases of sick (13%). All cases were followed up for 6 to 18 months, some residual recurrence in 4 cases, the rest are normal work or living self-care. Conclusion: Microsurgery is the main method for the treatment of AVM. It can improve the cure rate of AVM, reduce the complications, reduce the operation risk, and is safe and effective.
