目的探讨角膜塑形镜联合框架眼镜治疗青少年高度近视的临床效果及对裸眼视力及屈光度的影响。方法选取2015年9月-2016年9月盐城市第一人民医院眼科接诊的220例高度近视青少年患者为研究对象,按照随机数表法分为观察组(110例,209眼)和对照组(110例,211眼)。对照组仅佩戴框架眼镜,观察组在夜间佩戴角膜塑形镜、日间佩戴框架眼镜。于治疗前、治疗后1个月、3个月、6个月、1年时,比较两组裸眼视力、屈光度、眼轴长度、角膜内皮细胞密度、中央厚度的变化,并比较并发症的发生情况。结果观察组治疗后裸眼视力呈逐步上升趋势,对照组裸眼视力在治疗后6个月、1年时与治疗前比较均明显降低,差异有统计学意义(P<0.05),观察组在治疗后6个月、1年时,裸眼视力明显优于对照组,差异有统计学意义(P<0.05);观察组治疗后屈光度较治疗前明显降低,差异有统计学意义(P<0.05),对照组屈光度较治疗前均明显增高,差异有统计学意义(P<0.05),观察组在治疗后1个月、3个月、6个月、1年时,屈光度均明显低于对照组,差异有统计学意义(P<0.05);观察组治疗后眼轴长度与治疗前比较均无明显变化,差异无统计学意义(P>0.05),对照组在治疗后3个月、6个月、1年时,眼轴长度较治疗前比较明显延长,且均明显长于观察组,差异有统计学意义(P<0.05);两组治疗前后角膜内皮细胞密度、中央厚度比较,差异无统计学意义(P>0.05);两组并发症总发生率比较,差异无统计学意义(P>0.05)。结论在青少年高度近视患者中应用角膜塑形镜联合框架眼镜治疗效果显著,有助于改善裸眼视力及屈光度,安全性较高。
Objective To investigate the clinical effects of orthokeratology combined with spectacle lenses in the treatment of adolescent high myopia and its effect on uncorrected visual acuity and refraction. There was no significant difference in axial length between the observation group and before treatment (P> 0.05), while the control group showed no significant difference after 3 months, 6 months, At 1 year, the length of ocular axis was significantly longer than that before treatment, and both were significantly longer than those in observation group (P <0.05). There was no significant difference in corneal endothelial cell density and central thickness before and after treatment (P> 0.05). There was no significant difference in the overall incidence of complications between the two groups (P> 0.05). Conclusion The application of orthokeratology combined with spectacle lenses in the treatment of high myopia in adolescents is effective in improving visual acuity and refraction of uncorrected eyes and has higher safety.
