目的 评价趋势导向视野检查法 (tendency- oriented perimetry,TOP)在青光眼视功能检测中的应用价值。 方法 利用 Octopus10 1全自动视野计分别对 2 0名正常人的 2 0只正常眼、32例原发性开角型青光眼 (primary open- angle glaucoma,POAG)患者的 32只患眼和 14例可疑 POAG患者的 14只眼分别进行常规阈值视野检查 (Normal/Normal程序 )和 TOP视野检查 (TOP/Normal程序 )。所有受检眼在 2周内再次分别进行常规阈值视野检查和 TOP视野检查。将两种视野检查的结果、视野检查指数、点对点阈值变异及视野缺损点数进行比较和分析。 结果 TOP视野检查正常人的阴性检出率为 90 % ,POAG的阳性检出率为 75 % ,其中检测中晚期 POAG的阳性检出率达 10 0 %。 2种视野检查的视野指数呈明显正相关 ,平均敏感度 (mean sensitivity,MS)的相关系数为 0 .9335 ,平均缺损 (mean defect,MD)的相关系数为 0 .9189,偏离缺失 (loss variance,L V)的相关系数为 0 .96 2 1。点对点阈值变异和视野缺损点数 TOP视野检查结果略高于常规阈值视野检查结果 ,但二者间差异均无显著性的意义 (P=0 .2 0 19,P=0 .44 48)。 结论 TOP视野检查指数与常规阈值视野检查指数呈明显正相关 ,其检测中晚期 POAG的敏感性及可重复性高。
Objective To evaluate the value of trend-oriented perimetry (TOP) in the detection of glaucomatous visual acuity. Methods Twenty-two eyes of 32 normal primary open-angle glaucoma (POAG) patients and 20 suspicious cases of 20 normal eyes were enrolled in this study. Fourteen eyes of POAG patients underwent normal threshold visual field tests (Normal / Normal) and TOP visual field tests (TOP / Normal). All subjects underwent two kinds of examinations within two weeks, respectively, the conventional threshold visual field examination and TOP visual field examination. The results of the two kinds of visual field examinations, visual field index, point-to-point threshold variation and visual field defect points were compared and analyzed. Results The positive rate of POAG was 90% and the positive rate of POAG was 75% in TOP visual field. The positive rate of POAG was 100%. There was a significant positive correlation between the visual field indices of the two visual field examinations. The correlation coefficient of mean sensitivity (MS) was 0.9335, the correlation coefficient of mean defect (MD) was 0.9189, loss variance , LV) the correlation coefficient is 0.96 2 1. Point-to-point threshold variation and visual field defect TOP visual field test results slightly higher than the conventional threshold visual field test results, but there was no significant difference between the two (P = 0.2019, P = 0.4448). Conclusion TOP visual field index and normal threshold visual field index was positively correlated, the detection of late POAG is highly sensitive and reproducible.
