目的分析成纤维细胞生长因子诱导早期反应蛋白14(Fn14)在乳腺癌组织中的表达及其与临床病理因素和临床预后之间的关系。方法应用Western blot检测20例乳腺癌及癌旁正常乳腺组织中Fn14的表达差异。应用免疫组化染色检测Fn14在113例乳腺癌中的表达模式,并通过统计学分析Fn14与临床病理因素及临床预后之间的相关性。结果在通过Western blot检测的20组配对的乳腺癌及癌旁组织中,有17组的结果显示Fn14在乳腺癌中的蛋白表达水平明显高于癌旁正常乳腺组织。在113例进行免疫组化染色的乳腺癌中,Fn14阳性患者组与Fn14阴性患者组相比,HER2过表达(++/+++)患者比例同比HER2(-/+)患者比例明显增高(P=0.010);发生淋巴结转移的患者比例同比无淋巴结转移的患者比例明显增高(P=0.015);并且TNM分期Ⅲ期的患者比例同比Ⅰ期或Ⅱ期的比例也明显增高(P=0.030)。而与患者的年龄(P=0.436)、组织学类型(P=0.421)、组织学分级(P=0.790)、肿瘤大小(P=0.403)、ER(P=0.637)、PR(P=0.352)及Ki67指数(P=0.071)无关。通过Spearman相关性分析,我们发现Fn14与HER2的表达正相关(r=0.244,P=0.009)。另外,乳腺癌中Fn14的表达与肿瘤的预后显著相关(Log-rank=4.603,P=0.032)。结论 Fn14的高表达可能促进乳腺癌的恶性进展。Fn14可辅助用于乳腺癌的预后评估。
Objective To analyze the expression of Fn14 in breast cancer and its relationship with clinicopathological parameters and clinical prognosis. Methods Western blot was used to detect the expression of Fn14 in 20 cases of breast cancer and adjacent normal breast tissues. Immunohistochemical staining was used to detect the expression pattern of Fn14 in 113 cases of breast cancer. The correlation between Fn14 and clinicopathological parameters and clinical prognosis was analyzed. Results Of the 20 paired breast cancer tissues and paracancerous tissues detected by Western blot, 17 showed that Fn14 protein expression in breast cancer was significantly higher than that in adjacent normal breast tissue. In 113 cases of breast cancer with immunohistochemical staining, the proportion of HER2 (- / +) patients with HER2 overexpression (++ / +++) was significantly higher in Fn14-positive patients than in Fn14-negative patients (P = 0.010). The proportion of patients with lymph node metastasis was significantly higher than that without lymph node metastasis (P = 0.015). The proportion of patients with TNM stage Ⅲ in stage Ⅰ or stage Ⅱ was also significantly increased (P = 0.030) . (P = 0.436), histological type (P = 0.421), histological grade (P = 0.790), tumor size (P = 0.403), ER And Ki67 index (P = 0.071). By Spearman correlation analysis, we found a positive correlation between Fn14 and HER2 expression (r = 0.244, P = 0.009). In addition, the expression of Fn14 in breast cancer was significantly associated with tumor prognosis (Log-rank = 4.603, P = 0.032). Conclusion The high expression of Fn14 may promote the malignant progression of breast cancer. Fn14 can aid in the prognosis of breast cancer.
