目的探讨急性髓系白血病(AML)患儿免疫表型特征及其临床意义。方法采用流式细胞术四色免疫荧光直接标记技术与CD45/侧向角光散射(SSC)设门分析技术,检测98例AML患儿,根据抗体积分系统进行分型。结果 (1)AML 98例免疫表型分析显示,相关抗原的阳性强度表达依次为CD13、CD33、髓过氧化物酶,干/祖细胞分化抗原阳性强度表达依次为CD117、CD34。其中伴淋系抗原表达的阳性强度表达率依次为CD19、CD7、CD2、CD10。(2)98例分为伴淋巴系相关抗原的AML(Ly+AML)(n=38,38.78%)和不伴淋巴系相关抗原的AML(Ly-AML)(n=60,61.22%)。Ly-AML组首次化疗完全缓解(CR)11例(30.0%),Ly+AML组CR 18例(28.95%),2组CR率比较差异无统计学意义(P>0.05)。结论 AML患儿38.78%伴有淋巴系抗原表达,并以CD19的高表达为主,其次为CD7,其伴系抗原表达率的多少与AML患儿的CR率无关。
Objective To investigate the immunophenotype of children with acute myeloid leukemia (AML) and its clinical significance. Methods Flow Cytometry Four-color Immunofluorescence Direct Labeling and CD45 / Lateral-Angle Light Scattering (SSC) Assay to Detect 98 AML Children with AML were screened by antibody integration system. Results (1) The immunophenotypic analysis of 98 cases of AML showed that the positive expression intensity of related antigens was CD13, CD33, and the expression of myeloperoxidase and stem / progenitor differentiation antigen was CD117 and CD34. The positive expression rate of lymphoid antigen was CD19, CD7, CD2 and CD10. (2) 98 cases were divided into AML (Ly + AML) (n = 38,38.78%) with lymphoid-associated antigen and AML (Ly-AML) without lymphoid-related antigen (n = 60,61.22%). Eleven patients (30.0%) with complete remission (CR) in Ly-AML group and 18 (28.95%) patients in Ly + AML group had no significant difference in CR rate (P> 0.05). Conclusion 38.78% of children with AML are associated with lymphoid antigen expression, with high expression of CD19, followed by CD7. The frequency of the expression of the accessory antigen is not related to the CR rate of children with AML.
