目的评估患者治疗教育(therapeutic patient education,TPE)在儿童特应性皮炎(atopic dermatitis,AD)长期治疗管理中的作用,了解TPE对AD疾病严重程度、复发频率以及对患者生活质量的改善作用;评估以“AD教育课堂”为主的AD患者治疗教育形式的可行性。方法北京儿童医院皮肤科门诊诊治的298例2~12岁AD患儿按照随机分组原则分为实验组(intervention group,IG)和对照组(control group,CG),采用“AD教育课堂”的形式对实验组患儿及其护理人员实施患者教育,随访期限6个月,分别在实验开始时(T0)、3个月(T1)、6个月(T2)对患儿进行SCORAD评分评估并填写儿童及幼儿皮肤病生活质量指数调查问卷、皮肤护理-润肤剂使用认知调查问卷等,以评估TPE在AD长期治疗管理中的作用,判断此教育形式的有效性及可行性。结果实验组患儿的SCORAD评分、生活质量(quality of life,QoL)均有显著改善,护理人员对皮肤护理重要性的认知程度较前增加,实验组评估结果明显优于对照组,差异有统计学意义。结论 TPE在AD患儿长期治疗管理中发挥着重要作用,本研究采取的以“AD教育课堂”为主要形式的患者治疗教育形式是可行的,其可积极推广,但需不断完善。
Objective To evaluate patient treatment education (therapeutic patient education, TPE) role in children with atopic dermatitis (atopic dermatitis, AD) long-term treatment management, understand TPE severity of AD disease, relapse frequency and improvement of quality of life; Evaluate the feasibility of treatment education modalities for AD patients based on AD Education Classes. Methods A total of 298 AD children aged 2 ~ 12 years who were diagnosed and treated by Department of Dermatology, Beijing Children’s Hospital were divided into intervention group (IG) and control group (CG) according to the principle of randomization. , The patients were educated in the experimental group and their nursing staff. Conclusion TPE plays an important role in long-term treatment of children with AD management, this study take to “AD education classroom ” is the main form of treatment for patients feasible form of education, which can be actively promoted, but need to continue to improve.
