目的探讨体重指数(BMI)对COPD患者心肺运动耐力的影响。方法 70例稳定期COPD患者按照BMI指数分为超重组(27例)、正常体重组(20例)、和低体重组(23例)进行负荷连续递增的心肺运动试验(CPET),在运动中检测受试者最高运动状态时氧耗量(peakVO2)、最高运动状态时运动负荷(peakLoad)、最高运动状态时氧脉搏(peakO2pulse)、无氧阈(AT)和最高运动状态时的呼吸困难指数(peakVE/MVV)等各项参数。在运动前进行BMI指数评估和常规肺功能(PFT)检查。结果 (1)超重组和正常体重组COPD患者peakVO2、peakLoad、peakO2pulse和AT均高于低体重组,差异具有显著性(P<0.05);超重组和正常体重组COPD患者peakVE/MVV均低于低体重组,比较差异具有显著性(P<0.05)。(2)COPD患者BMI指数与peakVO2占预计值百分比、peakLoad占预计值百分比、peakO2pulse占预计值百分比、AT呈正相关(r=0.695,0.433,0.718,0.823,P均<0.01),与peakVE/MVV呈负相关(r=-0.451,P<0.01)。结论 BMI指数与COPD患者肌肉有氧代谢能力和运动耐力有关。
Objective To investigate the effect of body mass index (BMI) on cardiorespiratory exercise tolerance in COPD patients. Methods Seventy patients with stable COPD were divided into two groups: overweight group (27 cases), normal body weight group (20 cases) and low body weight group (23 cases) according to BMI index. Peak oxygen consumption (peakVO2), peakLoad at peak exercise, peak O2pulse, anaerobic threshold (AT) at peak exercise, and index of dyspnea at the highest exercise state were measured. (peakVE / MVV) and other parameters. BMI index assessment and routine pulmonary function (PFT) examination were performed prior to exercise. Results (1) The peakVO2, peakLoad, peakO2pulse and AT in overweight and normal weight COPD patients were significantly higher than those in low weight group (P <0.05). The peakVE / MVV in overweight and normal weight COPD patients were lower than Low weight group, the difference was significant (P <0.05). (2) The percentage of peakLoad to predicted value, peakO2pulse, percentage of predicted value and AT were positively correlated with BMI index and peakVO2 in COPD patients (r = 0.695,0.433,0.718,0.823, P <0.01) Was negatively correlated (r = -0.451, P <0.01). Conclusions The BMI index is related to muscle aerobic metabolism and exercise tolerance in COPD patients.
