目的:评价2.2 mm微切口晶状体超声乳化散光矫正型人工晶状体(toric IOL)植入术矫正白内障角膜散光患者的效果。方法:前瞻性随机对照研究。纳入南通大学第二附属医院2018年8月至2019年8月有角膜散光的白内障者56例(75只眼)。其中进行微切口超声乳化人工晶状体植入其中toric IOL植入术者29例(36只眼)为试验组;27例(39只眼)植入普通非球面IOL为对照组。术后随访6个月,观察对比两组术后视力、角膜散光对比敏感度及人工晶状体旋转稳定性。结果:随访末期,试验组裸眼视力优于对照组(n t=-2.964,n P=0.004),但两组之间最佳矫正视力相近(n t=-0.497,n P=0.621);试验组术后角膜散光(0.378±0.184)D较对照组(2.364±0.616)D明显减少(n t=-18.584,n P<0.001),而试验组术后角膜散光与术前预期残余散光(0.358±0.216)D之间的差异无统计学意义(n t=-0.580,n P=0.566);试验组的对比敏感度优于对照组(均n P<0.05)。术后两组轴位旋转差异无统计学意义(n F=1.076,1.247;n P=0.376,0.419)。n 结论:微切口超声乳化toric IOL植入术治疗有角膜散光的白内障安全有效。“,”Objective:To evaluate the efficacy of 2.2 mm microincision phacoemulsification with toric intraocular lens(IOL) implantation to correct corneal astigmatism in patients with cataract.Methods:This was a prospective random control study.A total of 75 eyes of 56 cases with cataract and corneal astigmatism from Aug. 2018 to Aug. 2019 in the Second Affiliated Hospital of Nantong University were randomly enrolled. All cases underwent microincision phacoemulsification with IOL implantation. In the trial group, 36 eyes of 29 cases were implanted with the toric aspherical IOL. In the control group, 39 eyes of 27 cases were implanted with the common aspherical IOL. These patients were followed up for 6 months. The postoperative visual acuity, corneal astigmatism, contrast sensitivity and IOL vatation were observed.Results:At the end of the follow-up, the uncorrected visual acuity of the trial group was better than that of the control group (n t=-2.964, n P=0.004), but the best corrected visual acuity was similar between the two groups(n t=-0.497, n P=0.621). The postoperative corneal astigmatism of the trial group (0.378±0.184)D was significantly lower than that of the control group (2.364±0.616)D (n t=-18.584, n P<0.001). In the trial group, there was no difference between preoperative predictive astigmatism and postoperative residual astigmatism (n t=-0.580, n P=0.566). The contrast sensitivity of the trial group was better than that of the control group (all n P<0.05). There was no significant difference in IOL rotation after operation in both groups (n F=1.076, 1.247; n P=0.376, 0.419).n Conclusion:Microincision phacoemulsification with toric IOL implantation is safe and effective for the treatment of cataract with corneal astigmatism.
