静脉血栓的起源与发生,是在免疫细胞平衡功能崩溃状态下,失去及时、有效清除胞内感染细胞/恶变细胞的功能,静脉内发生的感染性炎症/非感染性炎症。活化的血小板、白细胞膜整合素受体β3、β2与配体纤维蛋白原等免疫粘附性结合,在静脉内构建可逆性结合的丝状筛网结构,似静脉生物滤器,承担机体的代偿性物理防御,其功能是阻止静脉下端的胞内感染细胞/恶性肿瘤细胞回流全身,当以红细胞为主的血细胞回流滞满生物滤器内,红色血栓形成。可逆性结合的丝状筛网结构,是在血流滞缓状态下完成的,快速血流状态下,丝状筛网结构被是难以建立的。中性粒细胞膜整合素β2受体与配体Ⅹ结合,转化为Ⅹa,促使纤维蛋白原转变为纤维蛋白,静脉生物滤器变得牢固。获得性、家族性静脉血栓的起源与形成机制是相同的。免疫细胞平衡功能健全者,机体的胞内感染细胞/恶变细胞被及时、有效清除,机体不需要建立静脉生物滤器这样的物理屏障,因此,也不可能发生急性静脉血栓事件。
