目的分析山东省中西部农村25岁以上居民糖尿病(DM)及糖调节受损(IGR)的影响因素,为制订干预措施提供依据。方法采用多阶段分层随机抽样方法,共调查16 388人,进行问卷调查并测量身高、体重、腰围、血压和空腹血糖;空腹血糖在6.1~7.0 mmol/L者复做口服葡萄糖耐量(OGTT)试验。DM及IGR的诊断依据WHO1999年糖尿病诊断标准,采用单因素和多因素非条件Logistic回归,分别对DM和IGR的影响因素进行分析。结果单纯性空腹血糖受损(I-IFG)主要危险因素有DM家族史(OR=1.609,95%CI=1.088~2.379)、高血压家族史(OR=1.363,95%CI=1.112~1.670)、BMI(OR=1.216,95%CI=1.082~1.367)、高血压(OR=1.401,95%CI=1.154~1.701)、年龄(OR=1.116,95%CI=1.030~1.208);空腹血糖受损合并糖耐量受损(IFG/IGT)的危险因素有年龄(OR=1.207,95%CI=1.022~1.425)、高血压(OR=1.662,95%CI=1.095~2.524)、腰围(OR=1.668,95%CI=1.359~2.048);DM的主要危险因素有DM家族史(OR=3.048,95%CI=2.276~4.083)、年龄(OR=1.419,95%CI=1.313~1.534)、高血压(OR=1.407,95%CI=1.180~1.677)、腰围(OR=1.555,95%CI=1.425~1.698);另外还有职业、锻炼、收入。结论年龄、高血压、肥胖是IGR和DM的共同影响因素。老年人特别是患有高血压和中心性肥胖的老年人是预防的首要目标人群。
Objective To analyze the influencing factors of diabetes mellitus (DM) and impaired glucose regulation (IGR) in rural residents over the age of 25 in central and western Shandong Province, and to provide the basis for making interventions. Methods A total of 16 388 subjects were surveyed by questionnaire and height, weight, waist circumference, blood pressure and fasting blood glucose were measured by stratified random sampling method. Oral glucose tolerance (OGTT) was measured in patients with fasting blood glucose of 6.1-7.0 mmol / test. The diagnosis of DM and IGR was based on the WHO diagnostic criteria for diabetes in 1999. Univariate and multivariate non-conditional logistic regression were used to analyze the influencing factors of DM and IGR respectively. Results The main risk factors of simple I-IFG were family history of hypertension (OR = 1.609, 95% CI = 1.088-2.379), family history of hypertension (OR = 1.363, 95% CI 1.112-1.670) , BMI (OR = 1.216, 95% CI = 1.082-1.367), hypertension (OR = 1.401, 95% CI = 1.154-1.701), age (OR = 1.116, 95% CI = 1.030-1.208) The risk factors of IFG / IGT were age (OR = 1.207, 95% CI = 1.022-1.425), hypertension (OR = 1.662, 95% CI 1.095-2.524), waist circumference (OR = 3.048, 95% CI = 2.276 ~ 4.083), age (OR = 1.419, 95% CI = 1.313-1.534), high Blood pressure (OR = 1.407, 95% CI = 1.180-1.677), waist circumference (OR = 1.555, 95% CI = 1.425-1.698), and occupations, exercise and income. Conclusion Age, hypertension and obesity are the common factors affecting IGR and DM. The elderly, especially those with hypertension and central obesity, are the primary targets of prevention.
