报道1例正常泪腺及14例泪腺混合瘤的免疫组化研究结果。正常泪腺中,腺泡上皮各种抗体标记均为阴性,导管上皮细胞角蛋白标记为阳性,而位于腺泡及导管上皮细胞周围的肌上皮细胞肌动蛋白标记为强阳性。泪腺良性混合瘤中,管状结构的内层细胞及鳞状细胞 KER、EMA 及CEA 为阳性表达,而 VIM、ACT 及 S-100等均为阴性表达。与此相反,管状结构的外层细胞、基质内梭形多角形细胞、粘液基质及软骨样成分等,VIM、ACT,S-100等为阳性表达,KER,EMA 及 CEA 等多为阴性表达。依据本实验研究的初步结果,我们认为泪腺混合瘤中管状结构的外层细胞、基质中梭形多角形细胞等均为变异的肌上皮细胞。且这种细胞在粘液基质形成中起重要作用。
An immunohistochemical study of 1 case of normal lacrimal gland and 14 cases of mixed lacrimal gland tumor was reported. Normal lacrimal gland, acinar epithelial antibody were negative for all markers, ductal epithelial cells keratin-positive, while in the acinar and ductal epithelial cells around the myoepithelial actin labeled as strong positive. In benign mixed lacrimal gland tumor, the inner cells and squamous cells in tubular structure were positive for KER, EMA and CEA, but negative for VIM, ACT and S-100. In contrast, the outer cells of the tubular structure, spindle-shaped polygonal cells within the matrix, mucus matrix and cartilage-like components, VIM, ACT, S-100 and other positive expression, KER, EMA and CEA were mostly negative expression. According to preliminary results of this experimental study, we believe that the outer cells of the tubular structure and the spindle-shaped polygonal cells in the mixed tumor of the lacrimal gland are all mutated myoepithelial cells. And such cells play an important role in mucus matrix formation.
