目的调查妊娠期高血压孕妇妊娠晚期甲状腺功能异常状况,并分析其发生甲状腺功能异常的相关因素,探讨预防措施,为改善妊娠结局提供依据。方法选取该院2011年6月-2015年6月收治的849例妊娠期高血压孕妇为研究对象,检测甲状腺过氧化物酶抗体、促甲状腺素、游离三碘甲状腺原氨酸及游离甲状腺素进行甲状腺功能异常筛查,同时对其临床资料进行回顾性分析,采用单因素及logistic回归多因素分析妊娠期高血压孕妇妊娠晚期甲状腺功能异常的相关因素。结果 849例妊娠期高血压孕妇,甲状腺功能异常254例,占29.92%。其中亚临床甲状腺功能亢进39例,占15.35%;甲状腺功能亢进62例,占24.41%;亚临床甲状腺功能减退症86例,占33.86%;甲状腺功能减退症67例,占26.38%。Logistic回归多因素分析提示,年龄、流产史、碘摄入量、甘油三酯≥2 mmoL/L、胆固醇≥5 mmoL/L是妊娠期高血压孕妇妊娠晚期甲状腺功能异常的危险因素(P<0.05)。结论加强高危人群甲状腺功能检查,同时提高妊娠期高血压孕妇对甲状腺功能异常的正确认识,普及相关知识,以期有效预防甲状腺功能异常发生。
Objective To investigate the status of thyroid dysfunction in the third trimester of pregnancy in hypertensive pregnant women and to analyze the related factors of thyroid dysfunction and to explore the preventive measures to provide the basis for improving the outcome of pregnancy. Methods A total of 849 pregnant women with gestational hypertension admitted to our hospital from June 2011 to June 2015 were enrolled in this study. Thyroperoxidase, thyrotropin, free triiodothyronine and free thyroxine Thyroid dysfunction screening, at the same time its clinical data were retrospectively analyzed, univariate and logistic regression multivariate analysis of pregnancy-induced hypertensive pregnancy-induced thyroid dysfunction in late pregnancy related factors. Results 849 pregnant women with hypertensive pregnancy, 254 cases of abnormal thyroid function, accounting for 29.92%. Among them, 39 cases were subclinical hyperthyroidism, accounting for 15.35%; 62 cases were hyperthyroidism, accounting for 24.41%; 86 cases were subclinical hypothyroidism, accounting for 33.86%; 67 cases were hypothyroidism, accounting for 26.38%. Logistic regression multivariate analysis showed that age, history of abortion, iodine intake, triglyceride≥2 mmoL / L, and cholesterol ≥5 mmoL / L were risk factors for thyroid dysfunction in third trimester of pregnancy in hypertensive patients with gestational hypertension (P <0.05 ). Conclusions To strengthen thyroid function tests in high-risk groups and improve pregnant women with hypertensive gestational hypertension correct understanding of thyroid dysfunction, popularization of relevant knowledge, in order to effectively prevent thyroid dysfunction.
