目的探讨糖尿病肾病(DN)患者血清脂肪特异性丝氨酸蛋白酶抑制剂(Vaspin)水平变化及其与炎症因子的关系。方法选择2型糖尿病(T2DM)患者118例,按DN临床进展分期分组:糖尿病白蛋白尿正常组(DM组)38例、早期DN微量蛋白尿组39例(早期DN组)、临床DN大量蛋白尿组41例(临床DN组),以同期45例体检健康者为对照组。采用ELISA法检测四组血清Vaspin、肿瘤坏死因子α(TNF-α)、白细胞介素6(IL-6)、单核细胞趋化因子1(MCP-1)和IL-10水平,比较各组水平,并进行相关分析。结果与对照组比较,其他各组Vaspin、TNF-α、IL-6、MCP-1水平高(P均<0.05),DM组、早期DN组、临床DN组IL-6、TNF-α、MCP-1水平依次升高,Vaspin水平依次降低,各组间比较,P均<0.05。临床DN组IL-10水平高于对照组,其余各组间比较,P均>0.05。Vaspin与促炎因子IL-6、TNF-α、MCP-1呈负相关(r分别为-0.42、-0.42、-0.46,P均<0.05),与抗炎因子IL-10无关(r=0.21,P>0.05)。结论 DN患者血清Vaspin水平升高,其与血清促炎因子呈负相关。
Objective To investigate the changes of serum lipid-specific serine protease inhibitor (Vaspin) in patients with diabetic nephropathy (DN) and its relationship with inflammatory cytokines. Methods A total of 118 patients with type 2 diabetes mellitus (T2DM) were enrolled in this study. According to the clinical stages of DN, 38 cases were normal diabetic nephropathy group (DM group), 39 cases were early DN microalbuminuria group (DN group) Urine group of 41 cases (clinical DN group), with 45 cases of healthy subjects as control group. The serum levels of Vaspin, TNF-α, IL-6, MCP-1 and IL-10 in serum were detected by ELISA. Level, and related analysis. Results The levels of Vaspin, TNF-α, IL-6 and MCP-1 in the other groups were significantly higher than those in the control group (all P <0.05) -1 levels in turn increased, Vaspin levels in turn decreased, the comparison between the groups, P <0.05. Clinical DN group IL-10 levels higher than the control group, the rest of the groups were compared, P> 0.05. Vaspin was negatively correlated with IL-6, TNF-αand MCP-1 (r = -0.42, -0.42, -0.46, P <0.05 respectively), but not with anti-inflammatory cytokine IL- , P> 0.05). Conclusion Serum Vaspin levels in patients with DN are elevated, which are negatively correlated with serum proinflammatory cytokines.
