我院自1982年以来应用胸大肌肌皮瓣和胸大肌——肋骨——皮瓣一期修复头颈部癌切除后缺损6例,报告如下。 病例资料 本组6例病历摘要见附表,其中一例喉癌为喉全切除术后8日,因感染发生大咽瘘,于感染控制后,施行胸大肌肌皮瓣修复。3例术前行~(60)Co放射,剂量为4000～5200rad。除1例肌皮瓣坏死外,余肌皮瓣和胸大肌—肋骨—皮瓣均成活(图1)。术后咀嚼、吞咽、语言功能均正常,面形基本恢复正常(图2a、2
Our hospital has applied pectoralis major myocutaneous flaps and pectoralis major muscle-ribs- flaps for the primary repair of 6 cases of head and neck cancer after excision since 1982. The report is as follows. Case data The medical records of 6 cases in this group are shown in the attached table. One case of laryngeal cancer was treated on the 8th after total resection of the laryngeal sphincter due to infection. After the infection was controlled, the pectoralis major myocutaneous flap was repaired. Three patients received ~(60)Co radiation before surgery and the dose ranged from 4000 to 5200 rad. Except for one case of myocutaneous flap necrosis, the I-muscle flap and the pectoralis major-rib- flap were alive (Figure 1). Postoperative chewing, swallowing, and language functions were all normal and the facial shape returned to normal (Figure 2a, 2).
