目的 :探讨通过支气管肺泡灌洗液 (BALF)端粒酶活性检测诊断肺癌的实际价值。方法 :应用端粒酶重复序列扩增记录法改进方法对 3 0例肺癌患者的BALF进行端粒酶活性检测 ,并以 10例非肿瘤患者的BALF作为对照。结果 :肺癌组BALF中有 2 6例 ( 86.7% )表达阳性 ,非肿瘤组有 1例 ( 10 % )表达阳性 (P <0 .0 1)。肺癌组中小细胞癌表达阳性率为 81.8% ,非小细胞肺癌为 89.5 % ,两组间无显著差异。镜下见肿瘤病变者表达阳性率 95 .0 % ,未见肿瘤病变者 (周围型 )亦可达 70 .0 %。结论 :对肺癌经纤支镜取BALF测定端粒酶的活性阳性率高 ,特异性亦佳 ;对肺癌的诊断尤其是周围型者可能很有价值 ;扩增结果用银染方法显示较放射自显影同样敏感 ,且经济又无同位素污染。
Objective: To explore the practical value of detecting telomerase activity in bronchoalveolar lavage fluid (BALF) for the diagnosis of lung cancer. Methods: The telomerase activity of BALF from 30 lung cancer patients was detected by modified method of telomerase repeat amplification amplification. BALF of 10 non-tumor patients was used as control. Results: Twenty-six cases (86.7%) of BALF were positive in lung cancer group and 1 (10%) in non-tumor group (P <0.01). The positive rate of small cell carcinoma was 81.8% in lung cancer group and 89.5% in non-small cell lung cancer group, there was no significant difference between the two groups. Microscopically, tumor-positive patients showed a positive rate of 95.0%, no tumor lesions (peripheral type) up to 70.0%. Conclusion: The positive rate and specificity of telomerase activity in lung cancer with bronchofibroscopy and bronchoalveolar lavage fluid (BALF) are high, and the diagnosis of lung cancer may be of great value in peripheral type. The results of silver staining show that the positive rate of telomerase activity Development is equally sensitive and economical without isotopic contamination.
